髋臼中心化的全髋关节置换术治疗髋关节发育不良型骨关节炎的疗效探讨

Clinical Effect of Acetabular Centralization in Total Hip Arthroplasty for the Treatment of Osteoarthritis Caused by Developmental Dysplasia of the Hip

目的分析髋臼中心化的全髋关节置换术(THA)治疗髋关节发育不良型(DDH)骨关节炎的临床效果。方法选取我院2013年1月至2015年1月收治的70例(78髋)髋关节发育不良型骨关节炎患者,随机分为两组各35例。对照组患者采用常规THA术治疗,观察组患者采用髋臼中心化的全髋关节置换术治疗,观察两组患者的治疗效果。结果在术中出血量、术后引流量、下肢短缩、Harris评分、X轴、Y轴距离等指标方面,观察组均优于对照组(P<0.05)。结论针对髋关节发育不良型骨关节炎患者,髋臼中心化的全髋关节置换术可纠正旋转中心位置,妥善固定髋臼假体,改善髋关节功能,提高治疗效果。

Objective To analyze the clinical effect of acetabular centralization in total hip arthroplasty （THA） for the treatment of osteoarthritis caused by developmental dysplasia of the hip （DDH）. Methods A total of 70 cases （78 hips） of DDH-induced osteoarthritis admitted to our hospital from January 2013 to January 2015 were selected and randomly divided into two groups, with 35 cases in each group. The control group was treated with THA, while the observation group was given acetabular centralization in THA. The curative effects of two groups were observed. Results The observation group was superior to the control group in the intraoperative blood loss, postoperative drainage volume, lower limb discrepancy, Harris score, distances to axis X and distances to axis Y （P 〈0.05）. Conclusions Acetabular centralization in total hip arthroplasty （THA） for the treatment of osteoarthritis caused by developmental dysplasia of the hip （DDH） can correct the position of center of rotation, properly fix acetabular prosthesis, improve the hip fimction, and improve the therapeutic effect.
