植入式静脉输液港与经外周静脉穿刺中心静脉置管在老年住院患者中的应用

Application of Implantable Venous Access Ports and Peripherally Inserted Central Catheters in Elderly Inpatients

目的探讨植入式静脉输液港(IVAP)与经外周静脉穿刺中心静脉置管(PICC)在老年住院患者中的应用效果。方法选择我院160例老年住院患者,随机分为IVAP组和PICC组,每组80例。观察比较两组导管留置率、意外拔管率、患者生活质量及导管并发症发生率。结果 1第1个月IVAP组与PICC组的导管留置率比较无统计学差异(P>0.05);第3个月及半年后,IVAP组的导管留置率显著高于PICC组(均P<0.05)。2IVAP组的意外拔管率显著低于PICC组(P<0.05)。3IVAP组的平均Karnfsky评分显著优于PICC组(P<0.05)。4IVAP组的导管并发症发生率显著低于PICC组(P<0.05)。结论在老年住院患者静脉给药中,IVAP较之于PICC的留置时间更长,意外拔管率和导管并发症发生率更低,且患者生活质量更优。

Objective To investigate the application effects of implantable venous access ports （IVAP） and peripherally inserted central catheters （PICC） in the elderly inpatients. Methods A total of 160 cases of elderly inpatients were randomly divided into the IVAP group and the PICC group equally. The catheter indwelling rate, rate of accidental extubation, quality of life of patients and incidence of catheter complications were observed and compared between two groups. Results ①At first month, the catheter indwelling rate showed no statistical difference between two groups （P 〉0.05）; At third month and sixth month, the catheter indwelling rate of WAP group was significantly higher than that of PICC group （both P 〈0.05）. ②The rate of accidental extuhation of IVAP group was significantly lower than that of PICC group （P 〈0.05）. ③The average Kamfsky score of IVAP group was significantly higher than that of PICC group （P 〈0.05）. ④The incidence of catheter complications of IVAP group was significantly lower than that of PICC group （P 〈0.05）. Conclusions Compared with PICC, IVAP applied in elderly inpatients for intravenous administration has longer indwelling time, lower rate of accidental extubation and incidence of catheter complications, and better quality of life of patients.
