中青年男性冠心病患者 PCI 术后心理状态及影响因素的调查研究

Postoperative psychological state and influencing factors of young and middle-aged male patients with ;CHD after PCI

目的：了解中青年男性冠心病患者冠状动脉介入治疗（PCI）术后的心理状态及影响因素，为临床介入手术治疗患者进行心理干预提供依据。方法选择2015年4—6月接受 PCI 手术的中青年男性冠心病患者110例，采用非精神科住院患者心理状态评定量表（MSSNS）进行问卷调查。结果中青年男性冠心病患者 MSSNS 量表抑郁因子得分（13．00±2．83）分，低于国内常模；愤怒因子得分（10．38±2．73）分，高于国内常模，差异均有统计学意义（t 值分别为－2．593，4．586；P ＜0．05）。患者心理状态与年龄、职业、医疗费用占家庭总收入比例、血管病变等因素有关，差异具有统计学意义（F 值分别为0．013，5．491，4．938，4．620；P ＜0．05）。结论PCI 术后的中青年男性冠心病患者愤怒、焦虑情绪发生率较高，提示护理人员应重视患者心理反应，采取针对性的个体化心理疏导，提高疾病治疗效果和患者生活质量。

Objective To comprehend postoperative psychological state and influencing factors of young and middle-aged male patients with coronary heart disease （CHD）after percutaneous coronary intervention （PCI）,so as to provide basis for psychological intervention on patients treated by clinical interventional operations.Methods A total of 1 1 0 young and middle-aged male patients with CHD,who had been operated by PCI surgery from April and June 201 5,were selected and surveyed by a mental status scale in non psychiatric settings （MSSNS）.Results Scores of depression factor in the MSSNS scale of young and middle-aged male patients with CHD were （1 3.00 ±2.83）points,lower than the norm in China,while scores of anger factor were （1 0.38 ±2.73）points,higher than the norm in China （t =-2.593,4.586;P 〈0.05）.Psychological state of patients were relevant to age,occupation,proportion of medical costs in family income,vascular diseases, and many other factors （F =0.01 3,5.491 ,4.938,4.620;P 〈0.05）.Conclusions High occurrence rate of anger and anxiety on young and middle-aged male patients with CHD after PCI operation suggests nurses to attach importance to the patients′psychological reaction and to conduct targeted and individualized psychological counseling,so as to get better treatment effect and improve patients′quality of life.
