摘要
目的探讨3D VIBE增强扫描对MRTA阴性三叉神经痛的诊断价值。方法收集我院3D VIBE增强扫描的MRTA阴性三叉神经痛患者12例,均于西门子工作站行横轴位、冠状位及斜矢状位MPR,对责任血管行MIP,观察责任血管和三叉神经的关系,所有患者均经MVD手术证实。结果 1)12例患者中,3D VIBE增强扫描显示责任血管12例,显示率100%(12/12),其中压迫Ⅰ°7例,Ⅱ°2例,Ⅲ°3例,术中7例血管与神经接触,2例粘连,3例为压迫;2)12例患者中,10例为静脉压迫,占83.3%(10/12),其中压迫Ⅰ°7例,Ⅱ°2例,Ⅲ°1例,2例为小动脉、静脉复合性压迫,占16.7%(2/12),均为压迫Ⅲ度;3)MIP、MPR可立体显示责任血管及与三叉神经的三维关系。结论 3D VIBE增强扫描是MRTA阴性三叉神经痛磁共振检查的有效补充,为MVD手术提供全面的解剖学信息。
Objective To investigate the diagnostic value of 3D VIBE enhanced scanning in MRTA negative trigeminal neuralgia. Methods MRTA negative patients with trigeminal neuralgia were examined 3D VIBE enhanced scanning, 12 cases were line axial, coronal and oblique sagittal MPR in the Siemens workstation, the offending vessels were MIP. The relationship of the offending vessels and the trigeminal nerve were observed. All patients were confirmed by MVD surgery. Results 1 ) 3D VIBE enhanced scanning showed offending vessels in 12 cases, the display rate was 100% ( 12/12), including 7 cases of oppression Ⅰ degree, 2 degree Ⅱ, 3 cases Ⅲ degree, 7 cases of vascular surgery in contact with the nerve, 2 cases of adhesions, 3 cases of oppression; 2) 10 cases of vein were oppressed, accounting for 83.3% ( 10/12), including 7 cases of oppression Ⅰ degree, 2 degree Ⅱ, 1 case of grade Ⅲ, two cases were oppressed by small artery and vein, accounting for 16.7% (2/12), which were Ⅲ degree of oppression; 3 ) MIP, MPR can display stereoscopic offending vessels and three-dimensional relationship with the trigeminal nerve. Conclusion 3D VIBE enhanced scanning is an effective complement to MRTA negative trigeminal neuralgia, which can provide a comprehensive anatomical information for MVD surgery.
