儿童肺部感染程度与维生素D水平的相关性研究 被引量:10

摘要 目的探讨儿童肺部感染程度与维生素D水平的相关性。方法选取2012年2月-2015年11月于西安交通大学第一附属医院就诊的社区获得性肺炎(CAP)患儿124例,按照病情轻重分为重症肺部感染组(n=42)与普通肺部感染组(n=82),同时选择同期体检的正常儿童90例作为对照组。比较3组儿童的维生素D水平,并对儿童肺部感染程度与维生素D水平的关系进行探讨。结果对照组儿童、普通肺部感染组及重症肺部感染组患儿25-羟基维生素D3[25(OH)D3]水平逐渐降低,3组儿童25(OH)D3水平比较,差异有统计学意义(F=34.230,P<0.05);普通肺部感染组与重症肺部感染组患儿25(OH)D3水平比较,差异有统计学意义(LSD-t=15.353,P<0.05);普通肺部感染组患儿与对照组儿童25(OH)D3水平比较,差异有统计学意义(LSD-t=9.581,P<0.05);重症肺部感染组患儿与对照组儿童25(OH)D3水平比较,差异有统计学意义(LSD-t=22.721,P<0.05)。Spearman相关分析结果显示,儿童肺部感染程度与25(OH)D3水平呈显著负相关(r=-0.379,P<0.01,95%CI=-0.581^-0.124)。结论儿童肺部感染程度与维生素D水平密切相关。 Objective To explore the correlation between the severity of pulmonary infection and the vitamin D level in children. Methods A total of 124 pediatric patients treated at The First Affiliated Hospital to Xi'an Jiaotong University between February 2012 and November 2015 were included. All children, diagnosed as community-acquired pneumonia (CAP), were divided into two groups, i.e. severe group (n =42) and mild-to- moderate group (n = 82) according to the severity of CAP. Meanwhile, another 90 healthy children undergoing health checkup at the same hospital were included as the control group. The vitamin D level was compared a- mong the three groups, and the correlation between the severity of pulmonary infection and the vitamin D level was analyzed. Results The highest 25(OH)D3 level was observed in the control group and the lowest in the severe group, with significant differences among the three groups (F=34.230, P〈0.05). The 25(OH)D3 level was significantly lower in the severe group compared with the mild-to-moderate group (LSD-t = 15.353, P〈0.05) or the control group (LSD-t=22.721, P〈0.05); and that in the mild-to-moderate group was also significantly lower compared with the control group (LSD-t =9.581, P〈0.05). The Spearman's correlation a- nalysis showed that, the severity of pulmonary infection was negatively correlated with the 25 (OH)D3 level in children (r= -0.379, P〈0.01, 95% CI= -0.581 to -0.124). Conclusion The severity of pulmonary infec- tion is closely correlated with the vitamin D level in children.
