彩色多普勒超声在小儿先天性肠旋转不良诊断及指导治疗中的应用

Application of color Doppler ultrasound in the diagnosis and treatment of children with congenital intestinal malrotation

目的：探讨彩色多普勒超声在小儿先天性肠旋转不良诊断及指导治疗中的应用价值。方法选取徐州市儿童医院收治的临床怀疑小儿先天性肠旋转不良患儿60例作为研究对象。所有患儿均经彩色多普勒超声及消化道造影检查，消化道造影诊断为小儿先天性肠旋转不良合并肠扭转的患儿均经手术证实。以消化道造影结合临床作为诊断标准，分析彩色多普勒超声对小儿先天性肠旋转不良的诊断符合率。结果60例患儿中，彩色多普勒超声检查诊断肠旋转不良54例，消化道造影诊断56例，超声诊断符合率为96．42％；38例消化道造影提示合并肠扭转中超声诊断35例，38例患儿合并肠扭转均经手术证实，超声诊断符合率为92．1％。结论彩色多普勒超声对小儿先天性肠旋转不良诊断符合率较高，可有效检出合并有肠扭转的患儿，可为临床手术治疗提供准确、有效的指导，具有临床应用及推广价值。

Objective To evaluate the application of the color Doppler ultrasound in the diagnosis and treatment of children with congenital intestinal rotation.Methods A total of 60 children who were admitted into Xuzhou Children′s Hospital due to suspected congenital intestinal rotation were included in the current study.All patients were examined by the color Doppler ultrasound and gastrointestinal radiography.Based on the results of gastrointestinal radiography, con-genital intestinal malrotation patients with midgut volvulus were confirmed by surgery.According to the results of gastroin-testinal imaging and clinical data as diagnostic criteria, the diagnostic accuracy by the color Doppler ultrasound in the di-agnosis of congenital intestinal rotation patients were analyzed.Results Among the 60 patients, 54 children were diag-nosed with intestinal malrotation by the color Doppler ultrasonography and 56 children by gastrointestinal imaging.The accuracy of ultrasonic diagnosis was 96.42%.Among 38 patients with midgut volvulus diagnosed by gastrointestinal ima-ging and confirmed by surgery were 35 children who were diagnosed by ultrasound.The accuracy of ultrasonic diagnosis was 92.1%.Conclusions Patients with congenital intestinal malrotation can be effectively diagnosed by the color Doppler ultrasonography, especially those with midgut volvulus, which can be applied in clinical setting.
