血尿酸在老年2型糖尿病并发非酒精性脂肪性肝病患者肝纤维化进展中的预测价值 被引量:4

摘要 目的探讨血尿酸(UA)在老年2型糖尿病并发非酒精性脂肪性肝病患者肝纤维化进展中的预测价值。方法将385例老年2型糖尿病患者根据肝脏彩超结果分为肝纤维化组组195例和对照组190例。调查糖尿病病程,测量体重、身高;检测血小板计数(PLT)、白蛋白(ALB)、天冬氨酸转氨酶(AST)、丙氨酸转移酶(ACT)、血肌酐(Scr)、SUA;测定24 h尿量,计算UA清除率和UA排泄分数。肝纤维化组根据血UA分为高UA组42例和正常UA组153例,分别计算非酒精性脂肪性肝病纤维化评分。结果肝纤维化组高UA血症的患病率(21.5%)明显高于对照组(8.9%)(P<0.05);高UA组预测晚期肝纤维化进展率为35.7%,显著高于正常UA组(20.9%,P=0.047)。排除晚期肝纤维化率正常UA组与高UA组差异无统计学意义(15%vs 11.9%,P>0.05)。结论血UA在老年2型糖尿病并发非酒精性脂肪性肝病患者的肝纤维化进展中具有一定的预测价值,结合非酒精性脂肪性肝病纤维化评分,为高危人群的评估和防治提供依据。
