人工股骨头置换术治疗老年股骨粗隆间骨折的疗效分析 被引量:1

摘要 目的探讨人工股骨头置换术治疗老年股骨粗隆间骨折的疗效。方法本研究选取了65例股骨粗隆间骨折患者,按随机数字表法分为两组,观察组(32例)采用人工股骨头置换术,对照组(33例)采用股骨近端髓内钉(PFNA)治疗,比较两组患者围手术期指标、Harris评分。结果对照组手术时间和术中出血量优于观察组(P<0.05),但观察组术后下地负重时间明显较短(P<0.05);治疗后,两组Harris评分较治疗前均提高,且观察组评分明显高于对照组(P<0.05)。结论与PFNA相比,虽然人工股骨头置换术手术时间较长,术中出血量较多,但能减少患者住院时间,提高患者髋关节功能,值得临床推广。 Objective To evaluate the efficacy of artificial femoral head replacement for the treatment of the intertrochanteric fractures in the elderly. Methods Sixty-five patients with intertrochanteric fracture were randomly divided into observation group (32 cases, artificial femoral head replacement) and control group (33 cases, PFNA). The perioperative indicators, Harris ratings, of the two groups were compared. Results The operative time and blood lost of the control group were significantly better (P〈0.05), but weight-bearing time of the observation group was significantly shorter than that of the control group (P 〈0.05); after artificial femoral head replacement and PFNA treatment, both Harris scores of the two groups were significantly improved, and the observation group's Harris score was significantly higher (P〈0.05). Conclusion Although artificial femoral head replacement has longer operative time and more blood loss compared with PFNA, but it can reduce hospitalization time and improve hip function in patients, which has high clinical value.
