补脾益肺汤联合丹参川芎嗪注射液治疗慢性肺源性心脏病的临床疗效及安全性评价

Clinical effects of Nourishing Spleen and Lung decoction combined with Danshen- chuanxiongqin injection on the treatment of chronic pulmonary heart disease and its safety assessment

目的观察补脾益肺汤联合丹参川芎嗪注射液治疗慢性肺源性心脏病(CPHD)的临床疗效及安全性评价。方法将150例CPHD患者随机分为2组,均予常规治疗,对照组75例予丹参川芎嗪注射液治疗,治疗组75例在对照组治疗基础上加补脾益肺汤治疗。2组均14 d为1个疗程,治疗1个疗程。比较2组疗效,观察2组治疗前后心功能分级改善情况、肺功能指标变化。结果总有效率治疗组92.00%,对照组78.66%,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。2组治疗后Ⅱ、Ⅲ、Ⅳ级心功能改善情况均优于本组治疗前(P<0.01)。2组治疗后Ⅰ级心功能改善情况比较差异有统计学意义(P<0.05),治疗组优于对照组。2组治疗后心脏指数(CI)、肺动脉压、氧分压[p(O_2)]、二氧化碳分压[p(CO_2)]及血氧饱和度均较本组治疗前改善(P<0.05),且治疗组治疗后改善优于对照组(P<0.05)。结论补脾益肺汤联合丹参川芎嗪注射液治疗CPHD,能够明显改善患者心、肺功能,临床疗效确切,患者治疗依存性高,药物安全可靠。

Objective To observe the clinical effects and safety of Nourishing Spleen and Lung decoction combined with Danshen- chuanxiongqin injection on the treatment of chronic pulmonary heart disease（ CPHD）.Methods 150 patients with CPHD were randomly divided into two groups that received routine treatment. 75 patients in control group were treated by Danshen- chuanxiongqin injection,and 75 patients in treatment group were treated by Nourishing Spleen and Lung decoction on the basis of control group treatment. The treatment course was 14 d in two groups. The curative effect was compared between two groups. The cardiac functional grading and pulmonary function indexes before and after treatment were observed in two groups. Results The total effective rate in treatment group（ 92. 00%） was superior to that in control group（ 78. 66%,P〈0. 05）. There were statistical differences on the improvements of grade I cardiac function after treatment in two groups（ P〈0. 05）,and the improvements in treatment group were superior to control group. The cardiac function of grade II,III and IV after treatment were improved in two groups（ P〈0. 01）. The cardiac index（ CI）,pulmonary artery pressure（ PAP）,p（ O_2）,p（ CO_2）and oxygen saturation after treatment were improved in two groups（ P〈0. 05）,and the improvements in treatment group were superior to control group（ P〈0. 05）. Conclusion Nourishing Spleen and Lung decoction combined with Danshen- chuanxiongqin injection can significantly improve the cardio- pulmonary function in CPHD patients,has definite clinical effects,with high dependency,safe and reliable,worthy of clinical application.
