阿尔茨海默病伴发夜间谵妄的临床护理及多奈派齐疗效分析

Analysis of Clinical Nursing and Curative Effect of Donepezil for Alzheimer's Disease with Night Delirium

目的：探讨阿尔茨海默病（AD）伴发夜间谵妄的临床护理及多奈哌齐疗效分析。方法：选取2014年1月~2015年6月某院神经内科收治的160例阿尔茨海默病伴发夜间谵妄患者为研究对象,采用抽签随机法均分为观察组和对照组,对照组给予多奈哌齐片进行治疗,观察组在对照组基础上给予个性化护理,两组均持续治疗6周,观察护理前、后1周、4周、6周两组患者日常生活量表（ADL）评分变化情况,同时比较治疗前、后两组简易智能量表（MMSE）、蒙特利尔认知评估量表（MoCA）评分情况。结果：两组干预后ADL评分分别均治疗前显著降低（P〈0.05）,且观察组护理干预后1周、4周、6周ADL评分（30.01±4.88）分、（25.18±3.66）分、（20.08±2.18）分分别均较对照组下降显著（P〈0.05）。治疗后两组MMSE、MoCA评分均较治疗前显著升高（P〈0.05）,且治疗后观察组MMSE（19.13±5.01）分、MoCA（27.68±1.11）分分别均较对照组升高显著（P〈0.05）。结论：AD伴夜间谵妄患者给予个性化护理干预后患者ADL评分显著改善,多奈哌齐对AD伴夜间谵妄患者具有一定的临床疗效。

Objective： To investigate clinical nursing and curative effect of donepezil for Alzheimer＇s dis- ease with night delirium. Methods： 160 cases of Alzheimer＇s disease associated with night delirium patients treated in a hospital from January 2014 to June 2015 were taken as research objects, who were randomly di- vided into observation group and control group. The control group were treated with donepezil therapy, the observation group were treated with individual nursing on the basis of the treatment for the control group. Both the two groups were treated for 6 weeks, then compare daily living volume table （ADL） score changes, simple intelligent table （MMSE） and Montreal Cognitive Assessment Scale （MoCA） scores before and after the treatment at the 1st week, 4th week, 6th week of the two groups. Results. Prognosis ADL scores of the two groups after treatment were significantly lower that before treatment （P〈0.05） and observation group received nursing intervention. After 1W, 4W, 6W ADL score （30.01±488）, （25.18±366）, （20.08±2.18） respectively were lower than those in the control group decreased significantly （P〈0.05）. After treatment, two groups of MMSE, MoCA scores were significantly higher than those before treatment （P〈0.05）, and after treatment, the observation group MMSE （19.13±5.01）, MoCA （27.68±1.11） were significantly high- er than those of the control group （P〈0.05）. Conclusion. AD with night delirium were given individualized nursing intervention, the ADL score improved significantly and donepezil on AD with nocturnal delirium pa- tients has certain clinical curative effect.
