自发性气胸改良胸腔闭式引流术应用效果观察

Clinical observation on effects of modified closed thoracic drainage for spontaneous pneumothorax

目的探讨自发性气胸改良胸腔闭式引流术的应用效果。方法自发性气胸患者40例随机分为观察组和对照组,各20例。对照组给予常规胸腔闭式引流术,观察组施行改良胸腔闭式引流法,应用中心静脉留置导管行经皮胸膜腔置管闭式引流。评价临床疗效,对比2组疼痛评分、平均住院日、舒适度及护理服务满意度。结果 2组临床疗效差异无统计学意义(P>0.05)。观察组患者置管术后疼痛评分、平均住院日显著低于对照组(P<0.05或P<0.01);观察组患者心理舒适度和护理满意度均显著高于对照组(P<0.05或P<0.01)。结论自发性气胸患者应用中心静脉留置导管引流治疗,增加患者舒适度,提高护理服务满意度。

Objective To observe the clinical effect of modified closed thoracic drainage for spontaneous pneumothorax. Methods A total of 40 patients with spontaneous pneumothorax were ran- domly divided into the observation group and the control group, with 20 cases in each group. The con- trol group was given conventional closed thoracic drainage and the observation group was given modi- fied closed thoracic drainage with central venous indwelling catheter. The clinical effect, pain degrees, length of stay, mental comfort and nursing satisfaction were compared between two groups. Results There was no significant difference in clinical effect between two groups （P 〉 0.05 ）. The observation group had lower score in pain degree assessment after operation and shorter length of stay compared with the control group （ P 〈 0.05 or P 〈 0.01 ）. The patients in observation group received higher mental comfort and satisfaction degree to nursing service compared with those in the control group （P 〈 0.05 or P 〈 0.01 ）. Conclusion Application of modified closed thoracic drainage with central ve- nous indwelling catheter can increased the comfort level and satisfaction degree of patients with sponta- neous pneumothorax
