中西医结合治疗Ⅱ、Ⅲ度直肠脱垂疗效观察和护理探讨

Effects of the combined traditional Chinese and Western medicine in treatment of Ⅱ and Ⅲ degree rectal prolapse and related nursing management

目的探讨中西医结合治疗Ⅱ、Ⅲ度直肠脱垂的效果。方法选取Ⅱ、Ⅲ度直肠脱垂患者38例,选择中西医结合疗法,即采用直肠黏膜柱状缝合、点状注射加外括约肌折叠术联合中药内服治疗。结果 38例患者中2例失访,36例患者中治愈30例(83.3%),有效5例(13.9%),无效1例(2.7%)。患者术后均未发生直肠坏死、溃疡、出血、肛门失禁、切口感染等并发症。术后12例发生下腹部坠痛,经内服中药汤剂对症治疗后缓解。结论中西医结合治疗Ⅱ、Ⅲ度直肠有效,同时,围手术期优质护理、术后早期功能锻炼和健康指导对疾病康复具有积极意义。

Objective To investigate the effect of combined traditional Chinese and Western medicine in treatment of Ⅱ and m degree rectal prolapse. Methods This study included 38 patients with Ⅱ or Ⅲ degree rectal prolapse. All patients received combined traditional Chinese medicine and Western medical treatment, including longitudinal suture of mucosa, injection under the rectum mu- cous membrane and external sphincter folds. Results There were 2 cases lost to follow-up. Of 36 ca- ses evaluated for clinical effect, 30 cases were cured, 5 cases were improved and 1 case with ineffec- tive outcome. There was no patient with rectum necrosis, rectal mucous membrane ulcer, hemoproc- tia, anal incontinence or incision infection after surgery. A total of 12 patients with hypogastralgia were treated with traditional Chinese medicine decoction oral intake. Conclusion Combined traditional Chinese and Western medicine is an effective way in treatment of Ⅱ and Ⅲ degree rectal prolapse. High quality of nursing during perioperative period, functional training and health education after oper- ation are important to the rehabilitation of patients.
