免疫调节性细胞及其效应分子在CITP免疫功能紊乱中的作用研究

目的探讨调节性T细胞(Treg)、调节性B细胞(Breg)、辅助性T细胞17(Th17)、辅助性T细胞22(Th22)及其相关细胞因子的变化在慢性特发性血小板减少性紫癜(CITP)患者免疫功能紊乱中的作用。方法选择CITP患者40例、健康者40例,分离外周血单个核细胞,采用流式细胞术检测Treg、Breg、Th17、Th22细胞数量与比例,采用酶联免疫吸附法检测血浆中白细胞介素10(IL-10)、转化生长因子β1(TGF-β1)、白细胞介素17(IL-17)、白细胞介素(IL-22)水平,计算IL-17/IL-10、IL-22/TGF-β1比值。比较CITP患者、健康者各指标检测结果。结果 CITP患者Th17、Th22细胞及IL-17、IL-22水平高于健康者,Treg、Breg细胞及IL-10、TGF-β1水平低于健康者(P<0.05)。CITP患者Th17/Treg、Th22/Breg、IL-17/IL-10、IL-22/TGF-β1比值水平均高于健康者(P<0.05)。结论 Th17、Treg、Th22、Breg细胞水平异常及IL-17/IL-10、IL-22/TGF-β1比值变化可能与CITP患者免疫功能紊乱密切相关。

Objective To study the effect of T helper cells 17（Th17）,T helper cells 22（Th22）,regulatory T cells（Treg）,regulatory B cells（Breg）and related cytokines on immune disorder in patients with chronic idiopathic thrombocytopenic purpura（CITP）.Methods A total of 40 CITP patients and 40 healthy subjects were enrolled and detected for levels of Treg,Breg,Th17,Th22,and levels of interleukin-17（IL-17）,interleukin-22（IL-22）,interleukin-10（IL-10）and transforming growth factorβ1（TGF-β1）.The ratios of IL-17/IL-10 and IL-22/TGF-β1were analysed.Results Levels of Th17,Th22,IL-17 and IL-22 in CITP patients were significantly higher than healthy subjects,but levels of Treg,Breg,IL-10 and TGF-β1were lower（P〈0.05）,The ratios of Th17/Treg,Th22/Breg,IL-17/IL-10 and IL-22/TGF-β1in CITP patients were higher than healthy subjects（P〈0.05）.Conclusion Abnormality of levels of Th17,Th22,Treg,Breg,and changes of IL-17/IL-10 and IL-22/TGF-β1ratios might play an important role in the immune diorder in CITP patients.
