无创呼吸机辅助通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的疗效分析

Efficacy of Non-invasive Mechanical Ventilation in Chronic Obstructive Pulmonary Disease With Type Ⅱ Respiratory Failure

目的探讨无创呼吸机辅助通气治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的疗效。方法选取40例COPD合并Ⅱ型呼吸衰竭患者作为研究对象,在给予吸氧、祛痰、抗感染、纠正酸碱失衡、电解质紊乱、营养支持等常规治疗措施的基础上使用无创正压通气,观察治疗前后受试患者呼吸频率、血气分析变化。结果治疗后,患者动脉血气分析p H、PO2、SO2较治疗前升高,差异有统计学意义(P<0.05),PCO2、RR均降低(P<0.05)。结论使用无创呼吸机治疗COPD合并Ⅱ型呼吸衰竭,能减轻患者呼吸肌疲劳,纠正氧气及二氧化碳潴留,改善血流动力学,且操作简单,使用方便,是一种快速、有效、安全的治疗方法。

Objective To study the efficacy of non-invasive mechanical ventilation in COPD with type Ⅱ respiratory failure. Methods Patients with COPD complicated with type Ⅱ respiratory failure were taken as research subjects,on basis of oxygen,expectorant,anti-infection,correct acid-base imbalance,electrolyte imbalance,nutritional support and other conventional treatment,they were given noninvasive positive pressure ventilation,after the therapy,the blood gases,respiratory rate were observed. Results p H and Oxygen of the patient compared with prior treatment were statistically significant（P 〈0.05）,PCO2,SO2,RR were significantly lower（P 〈0.05）. Conclusion Non-invasive mechanical ventilation treatment of COPD with type Ⅱ respiratory failure patients can reduce respiratory muscle fatigue,correct Oxygen and carbon dioxide retention,improve blood flow dynamics,and the operation is simple,easy to use,it is a fast,effective and safe treatment.
