广州市儿童医疗保健体系现状分析

Analysis of Status of Children Medical Healthcare System in Guangzhou

目的分析广州市儿童人群医疗保健服务的需求以及需求与现状的差距,提出加强广州市儿童医疗保健服务体系建设的策略与措施,为政府提供决策依据和政策支持。方法该课题主要采取问卷调查法,对广州市十二个区域的一定数量的儿童医疗保健机构的一定数量的入院患儿家属、一般医护人员和对相应辖区内的居民进行问卷调查,并结合相关文献进行讨论与思考。结果三级医院聚集了大量高职称、高学历医务人员,而一级、二级医院面临正高职称医务人员严重不足;医护比例配置不尽合理,12个区平均医护比为1:1.26;全市仅有广州市儿童医院和越秀区儿童医院2所儿童专科医院,但其承担了全市将近一半的儿童诊疗服务量;每千儿童医生数0.89名,与资料查询的北京每千儿童医生数1名,仍存在一定差距。结论儿童医疗保健体系的人力资源情况仍不能满足民众的需求,恢复儿科医生本科培养体系才是长远之计。应加大力度引进人才,利用高校-高校附属医院-高校附属医院规培基地功能,加强儿科医生培养力度。不断探索补偿机制,建立基于床位对儿科专项补助的补偿机制。

Objective To analyze the demands of children medical healthcare system in Guangzhou and distance between the demands and status, and put forwards the strategies and measures of enhancing the construction of children medical healthcare service system in Guangzhou and provide decision-making support and policy support for governments. Methods The questionnaires were mainly adopted in this topic, and a certain quantity of admission children＇s family, general medical staff and residents in the corresponding regions in a certain children medical healthcare institutions in 12 regions in Guangzhou were surveyed by questionnaires, and the discussion and thought were conducted combined with the related literatures. Results According to the results show that three hospitals gathered a large number of high title, highly educated medical personnel, and primary, secondary hospitals is high title facing serious shortage of medical personnel. Configuration is not reasonable proportion of health care, health care than the average of the 12 districts is 1： 1.26. In addition, the city just have two children hospitals likes Guangzhou Children＇s Hospital and Yuexiu District Children＇s Hospital. But its commitment to the city for nearly half of the amount of treatment services for children. The survey shows that the number of doctors per thousand children 0.89, and data queries Beijing number of doctors per thousand children one, there are still some gaps. Conclusion The human resources in the children medical healthcare system still fails to satisfy the public demands, and recovering the undergraduate education system of doctors in the department of paediatrics is a long-term solution, and we should enhance the education force of doctors in the department of paediatrics, constantly explore the reimbursement mechanism and establish the reimbursement mechanism of specific grant in the department of paediatrics based on beds.
