西安市6家二级医院Ⅰ类切口手术预防用药干预分析

Analysis on prophylactic antibacterial use intervention for typeⅠincision operation in 6 secondary hospitals of Xi′an

目的分析西安市6家二级医院抗菌药物专项整治干预前后Ⅰ类切口手术预防用药情况,为基层医院Ⅰ类切口手术预防用药的合理使用提供依据。方法选择西安市6家二级医院共计3 235例Ⅰ类切口手术病例,分析其预防用药情况,结合抗菌药物整治工作所采取的干预措施,分析干预效果。结果Ⅰ类切口手术抗菌药物预防使用率从87.8%下降为34.0%,预防时机合理率上升到95.6%,预防给药疗程合理率上升到36.5%,给药品种选择合理率上升到53.3%,联合用药率下降到1.1%,预防给药完全合理率上升到22.4%,手术切口感染率未增加。结论西安市6家二级医院Ⅰ类切口手术预防用药情况得到明显改善,抗菌药物专项整治和干预对Ⅰ类切口手术起到了积极的推动作用。

Objective To analyze the prophylactic use of antibacterial agents before and after the intervention with type Ⅰ incision operations of 6secondary hospitals in Xi′an and to provide a reference for clinical rational drug use for primary hospitals.Methods3 235typeⅠincision cases of 6secondary hospitals in Xi′an were selected and analyzed.To combine with the national antimicrobial rectification work and interventions,the remediation intervention effect was analyzed.Results The antibacterial drugs use rate of typeⅠincision was declined from 87.8% to 34.0%,the reasonable prophylaxis timing rate rose to 95.6%,the reasonable prophylaxis course rate rose to 36.5%,the reasonable drug for prophylaxis rose to 53.3%,combination rate was declined to1.1%,and the entirely reasonable rate rose to 22.4%.There was no increase in surgical incision infection rate.Conclusion TypeⅠincision surgical prophylaxis in Xi′an 6secondary hospital was improved greatly,and the special rectification intervention for rational use of typeⅠincision operation prophylaxis has played a positive role.
