消肿退瘀汤改善肱骨髁间骨折患者肘关节功能临床研究

Clinical Research on the Function of Elbow in Patients with Humeral Intercondylar Fracture Improved by Swelling- Relieving and Blood-Stasis-Eliminating Decoction

目的：观察消肿退瘀汤改善肱骨髁间骨折患者肘关节功能的临床疗效。方法：选取2015年2月—2016年4月本院收治的肱骨髁间骨折患者136例,按照随机数字表法分为对照组和观察组,每组68例。对照组给予尺骨鹰嘴截骨入路术治疗,观察组在对照组治疗的基础上加用消肿退瘀汤治疗。结果：对照组有效率为77.9%,观察组有效率为92.6%,观察组疗效优于对照组（P〈0.05）;观察组治疗后中医证候积分优于对照组（P〈0.05）;观察组术后肘关节功能优良率为95.6%,对照组优良率为83.8%,观察组疗效优于对照组（P〈0.05）;观察组治疗后骨折处骨组织TGF-β、BMP-2、VEGF水平优于对照组（P〈0.05）;观察组并发症发生率低于对照组（P〈0.05）;观察组骨折的平均愈合时间明显短于对照组（P〈0.05）。结论：消肿退瘀汤能显著改善肱骨髁间骨折患者的临床症状,提高术后肘关节功能,并发症少,骨折愈合快,其机制与提高骨折处骨组织TGF-β、BMP-2、VEGF的表达以促进骨折愈合有一定的相关性。

Objective：To observe the clinical effects of Swelling-Relieving and Blood-Stasis-Eliminating Decoction on the improve- merit of humeral intercondylar fracture. Methods ：Totally 136 patients with humeral intercondylar fracture treated in this hospital between February 2015 and April 2016 were randomly divided into control group and observation group, with 68 cases in each group. Control group were given osteotomy of the olecroanon while observation group were added with Swelling-Relieving and Blood-Stasis-Eliminating Decoction. Results：The effective rate was 77.9% in control group and 92.6% in observation group;observation group were better than control group （ P 〈 0.05 ）. The levels of TGF-β, BMP-2 and VEGF in the facture area in observation group were better than these in control group（ P 〈 0.05 ）. The average healing time of the fracture in observation group was markedly shorter than that in control group （ P 〈 0. 05 ）. Conclusion ： Swelling-Relieving and Blood-Stasis-Eliminating Decoction can significantly improve the clinical symptoms of humeral intercondylar fracture and enhance the function of the elbow after surgery with few complications and rapid healing of the fracture;the mechanism has certain relativity with the expression of TGF-β, BMP-2 and VEGF.
