136例肝癌肿瘤标志物特点分析

Analysis on characteristics of tumor markers in 136 cases of liver cancer patients

目的通过联合测定病人血清中的5项肿瘤标志物,分析肝癌的肿瘤标志物表达特点,探讨肿瘤标志物联合检测对肝癌的诊断意义。方法选取2014年8月至2015年9月于盐城市第一人民医院进行AFP、CEA、CA199、CA125Ⅱ、CA724五项肿瘤标志物检测的患者,包括肝癌患者136例,肝硬化患者106例,慢性肝炎患者117例,健康体检者102例,分析其肿瘤标志物表达水平及阳性率,计算并分析单项检测和联合检测的灵敏度及特异度。结果肝癌组AFP表达水平和阳性率显著高于其他三组;肝癌组CEA、CA199、CA125Ⅱ表达水平和阳性率显著高于慢性肝炎组和健康体检组,而与肝硬化组之间无显著差异;肝癌组CA724的表达水平低于健康体检组,各组阳性率无显著差异;AFP、CEA、CA199、CA125Ⅱ四项联合检测肝癌的灵敏度和特异度均较高。结论 AFP、CEA、CA199、CA125Ⅱ四项联合检测可用于肝癌的辅助诊断,CA724不适用于对肝癌的诊断。

Objective To investigate the characteristics of tumor markers in liver cancer patients through determination of five serum tumor markers,and explore the significance of joint detection of tumor markers in diagnosis of liver cancer. Methods We chose the patients and healthy cases,who carried out detection tumor markers including AFP,CEA,CA199 and CA125 Ⅱ and CA724 in the first people ＇ s hospital of Yancheng between August 2014 and September 2015,as research objects. There were 136 cases of liver cancer patients,106 cases of patients with liver cirrhosis,117 cases of patients with chronic hepatitis,and 102 cases of health persons. We analyzed the expression levels and positive rate of the tumor markers,and calculated the sensitivity and specificity of single detection and joint detection. Results AFP expression level and positive rate in liver cancer patients was significantly higher than other three groups. CEA,CA199 and CA125Ⅱ expression levels and positive rates in liver cancer patients was significantly higher than that of chronic hepatitis group and healthy control,but has no significant difference compared with liver cirrhosis group. Expression level of CA724 in liver cancer group was lower than the healthy control group,and the positive rate has no significant difference among all groups. Sensitivity and specificity of AFP,CEA,CA199 and CA125Ⅱ joint detection on liver cancer relatively high.Conclusions Joint detection of AFP,CEA,CA199 and CA125 can be applied in the diagnosis of liver cancer,CA724 is not applicable to the diagnosis of liver cancer.
