当归黄芪饮联合西药辨治难治性高血压随机平行对照研究

Danggui Huangqi Yin(当归黄芪饮) combined with Western Medicine in Treating Refractory Hypertension Randomized Controlled Study

[目的]观察当归黄芪饮联合西药治疗难治性高血压疗效。[方法]使用随机平行对照方法,将90例住院及门诊患者按掷骰子方法简单随机分为两组。对照组45例美托洛尔50mg/次,2次/d;贝那普利10mg/次,2次/d;氢氯噻嗪20-50mg/次,1次/d;硝苯地平30-60mg/次,1次/d。治疗组45例当归黄芪饮,玉米须60g,天麻10g,当归、黄芪各10g,1剂/d,水煎服400m L,早晚口服;西药治疗同对照组。连续治疗15d为1疗程。观测临床症状、尿素氨、尿微量蛋白、24h尿蛋白定量、肌酐、血红蛋白、血浆蛋白、不良反应。治疗1疗程（15d）,判定疗效。[结果]治疗组痊愈12例,显效23例,有效8例,无效2例,总有效率96.32%;对照组痊愈2例,显效3例,有效7例,无效30例,总有效率33.25%;治疗组疗效优于对照组（P〈0.01）。尿素氨、尿微量蛋白、24h尿蛋白定量、肌酐、血红蛋白、血浆蛋白两组均有改善（P〈0.05,P〈0.01）,治疗组改善优于对照组（P〈0.05,P〈0.01）。[结论]当归黄芪饮联合西药治疗难治性高血压,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the curative effect of angelica astragalus decoction combined with western medicine in the treatment of refractory hypertension. [Methods] 90 patients were randomly divided into two groups according to the method of random parallel control. The control group of 45 cases of metoprolol 50rag once, twice daily; benazepril 10 mg/time, 2 times daily; hydrochlorothiazide 20-50mg, 2 time/d; nifedipine 30-60 mg/time, 1 times/d. In the treatment group, 45 cases of angelica astragalus drink, corn to 60g, 10g angelica, astragalus each 10g, 1 agent/d, water decoction 400mL, sooner or later oral; western medicine treatment with the control group. Continuous treatment for 1 courses of 15d. Observation of clinical symptoms, urea, urine trace protein, 24 hours urinary protein quantitation, creatinine, hemoglobin, plasma protein, adverse reactions. Treatment of 1 courses of treatment （15 d）, determine the efficacy. [Result] In the treatment group, 12 cases were cured, 23 cases were markedly effective, effective in 8 cases, ineffective in 2 cases, efficiency 96.32%; in the control group, 2 cases were cured, 3 cases were markedly effective, effective in 7 cases, invalid in 30 cases, efficiency 33.25%; the treatment group curative effect surpasses the control group （P〈0.01）. Urinary protein, urinary protein, 24h urinary protein, creatinine, hemoglobin, plasma protein two groups were improved （P〈0.05, P〈0.01）, the treatment group than the control group （P〈0.05, P〈0.01）. [Conclusion] Angelica Astragalus decoction combined with western medicine in the treatment of refractory hypertension, curative effect satisfaction, no serious adverse reaction, it is worth promoting.
