针灸联合康复治疗中风偏瘫随机平行对照研究

Randomized Parallel Controlled Study of Acupuncture combined with Rehabilitation Treatment for Stroke Hemiplegia

[目的]观察针灸联合康复治疗中风偏瘫疗效。[方法]使用随机平行对照方法,将80例住院患者按就诊顺序编号法简单随机分为两组。对照组40例康复治疗,借助临床设备或推拿手法锻炼患者肌肉,改善肌肉软组织粘连,缓解疼痛度,2-3次/d。治疗组40例针灸治疗,主穴：三阴交、内关、人中;下肢体偏瘫,取足三里、承山、绝骨;上肢偏瘫,取大椎、肩井、合谷、天宗,留针30min/次,1次/d,2周后停留3d再次针灸;康复治疗同对照组。连续治疗30d为1疗程。观测临床症状、Fugl Meyer运动功能评分、不良反应。治疗1疗程,判定疗效。[结果]治疗组治愈20例,好转18例,无效2例,总有效率95.00%;对照组治愈10例,好转22例,无效8例,总有效率80.00%;治疗组疗效优于对照组（P〈0.05）。生活能力、运动功能评分两组均有改善（P〈0.01）,治疗组改善优于对照组（P〈0.05,P〈0.01）。[结论]针灸联合康复治疗中风偏瘫,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the curative effect of acupuncture combined with rehabilitation therapy for stroke hemiplegia. [Method] using random parallel control method, 80 cases of hospitalized patients according to the order number method were randomly divided into two groups. The control group with rehabilitation treatment in 40 cases, with the help of clinical equipment or massage exercise for patients with muscle, improve muscle and soft tissue adhesions, relieve the pain degree of 2-3/d. Treatment of 40 cases of the treatment group of acupuncture and moxibustion, main points： Sanyinjiao （SP6）, Neiguan, people; lower limb hemiplegia, Toriashikaga Misato, Chengshan, bone; hemiplegic upper limb, Dazhui, Jianjing, Hegu, Tian Zong, needle retention 30 rain/times, 1 times/d, 2 weeks after stay 3d again acupuncture; rehabilitation treatment with the control group. Continuous treatment for 1 courses of 30d. Observation of clinical symptoms, blood biochemical indicators, FuglMeyer motor function score, adverse reaction. Treatment of 1 courses of treatment, determine efficacy. [Result] in the treatment group, 20 cases were cured, 18 cases improved, 2 cases ineffective, the total effective rate was 95%. The control group 10 cases were cured, 22 cases improved, 8 cases ineffective, the total efficiency of 80%. The treatment group than the control group （P〈0.05）. Life ability, motor function score of two groups were improved （P〈0.01）, the treatment group was better than the control group （P〈0.05,P〈0.01）. [Conclusion] Acupuncture combined with rehabilitation treatment of stroke hemiplegia, satisfactory curative effect, no serious adverse reactions, worthy of promotion.
