摘要
目的评价Loop抗返流双J管对宫颈癌放疗合并梗阻性肾病的临床治疗效果。方法收集本院2008年6月至2014年6月就诊的28例宫颈癌放疗合并梗阻性肾病患者,其中13例患者采用Loop抗返流双J管置入治疗、15例采用普通双J管置入治疗,定期复查尿常规、肾功能及B超,抗返流双J管留置1年,普通双J管每3月更替1次,随访1年,比较2组梗阻性肾病的缓解率、上尿路感染发生率、相关并发症发生率和满意度。结果所有患者随访时间均满1年,与置管前比较,2组患者肾积水程度均明显减轻,术后肾功能水平明显改善,差异无统计学意义(P>0.05)。Loop抗返流双J管组和普通双J管组1年内上尿路感染发生率、相关并发症发生率、满意度分别为7.7%和26.7%、15.4%和53.3%、84.6%和66.7%,差异均有统计学意义(P<0.05)。结论 Loop抗返流双J管对宫颈癌放疗合并梗阻性肾病的治疗效果确切,同时减少上尿路感染,相关并发症发生率低、满意度高,值得在临床推广。
Objective To assess the efficacy of the Loop antirefluxing D-J tube in obstructive nephropathy in the course of radiotherapy for cervical cancer. Methods A total of 28 cases of obstructive nephropathy in the course of ra- diotherapy for cervical cancer, admitted to the Department of Radiotherapy from June 2008 to June 2014, were treated with D-J tube.Thirteen patients were inserted with Loop antirefluxing D-J tube while 15 cases were treated with ordinary D-J tube. The follow-up information on the therapeutic effect was collected by ultrasonic testing and renal function tests and so on. The upper urinary tract infection rate, complication rate and satisfaction were compared between the two groups. Re- suits All the patients were followed up for 1 year. The degrees of hydronephrosis were mitigated for all patients while cre- atinine and urea nitrogen levels significantly improved after the operation, there were no significant differences between two groups (P〉0. 05). The upper urinary tract infection rate, complication rate and satisfaction of two groups were 7.7% (1/13) vs 26.7% (4/15), 15.4% (2/13) vs 53.3% (8/15), 84.6% (11/13) vs 66.7% (10/15), and there were significant differences be- tween two groups (P〈0. 05). Conclusion The Loop antirefluxing D-J tube has a good effect on obstructive nephropathy in the course of radiotherapy for cervical cancer and it can reduce the incidence of upper urinary tract infection and other complications, and get a high degree of satisfaction.
