糖皮质激素导致精神症状影响因素分析

Study on the influencing factors of mental symptoms caused by glucocorticoid

目的探讨影响糖皮质激素导致患者精神障碍的因素,为糖皮质激素临床应用提供指导。方法选取2012年1月至2015年6月接受糖皮质激素治疗560例患者进行研究,观察患者基本情况与精神障碍发生率的关系。结果本研究中糖皮质激素应用后精神症状的发生率为23.93%。单因素分析显示,性别、职业、学历、原发病类型等对精神障碍的发生率均无影响(P>0.05),年龄、性格、激素类型、激素剂量、激素应用时间、BMI等均可影响精神障碍的发生率(P<0.05)。多因素Logistic分析显示,激素使用时间、激素类型和激素剂量是导致精神障碍的危险因素(P<0.05)。结论糖皮质激素治疗可导致精神障碍,激素类型、激素剂量、激素应用时间等是危险因素,应引起临床医生的重视。

Objective To study the influencing factors of mental symptoms caused by glucocorticoid, to provide guidance for the clinical application of glucocorticoid. Methods 560 patients treated with glucocorticoid were selected as objects from January 2012 to June 2015. The relationship between general conditions and incidence of mental disorders of patients were observed and analyzed. Results The incidence of mental disorders caused by glucocorticoid was 23.93% in this study. Univariate analysis showed that gender, occupation, educationand types of primaryillness made no difference on the incidence of mental disorders （P 〉 0.05）, while, age, personality, glucocorticoid type, glucocorticoid dosage, glucocorticoid application time, BMI and so on can affect the incidence of mental disorders（P〈O.05）. M uhivariate Logistic analysis showed that the glucocorticoid application time, glucocorticoid types and glucocorticoid dose were the risk factors leading to mental disorder （P〈 0.05）. Conclusion Glucocorticoid treatment can lead to mental disorders, glucocorticoid types, glucocorticoid doses and glucocorticoid application time are the risk factors, which should be paid much more attention to by the clinicians.
