腹腔镜在军人急性阑尾炎中的应用价值

Value of Laparoscopic Appendectomy for Acute Appendicitis in Soldiers

目的比较腹腔镜阑尾切除术与传统开腹手术在军人急性阑尾炎治疗中的价值,并进行费用分析。方法对2014年1月至2015年12月间医院手术治疗的军人急性阑尾炎患者111人随机分为腹腔镜组和开腹手术组,收集在院和随访数据,进行统计分析。结果腹腔镜组在手术时间、下床时间、通气时间、术后住院时间、并发症、手术满意度方面均优于开腹手术者,尤其是院外术后半休时间(12.53±2.77天比25.95±4.98天,t=15.577,P<0.001)差别显著,但腹腔镜组治疗费用较高。结论腹腔镜手术较传统手术具有很多优点,在军人患者中可以有效降低非战斗减员,管理上可采用适当措施控制治疗费用。

Objective To compare laparoscopic appendectomy with conventional open surgery for the treatment of acute appendicitis in soldiers ,and to analyze the cost.Methods In this study,111 military patients di-agnosed as acute appendicitis between January ,2014 and December,2015 were randomly divided into two groups （laparoscopic appendectomy and conventional operation ）.The in hospital and followed -up data were collected. Statistical analysis was carried out .Results The laparoscopic appendectomy group were highly superior than con -ventional open operation group in the operation time ,ambulation time, bowel move time, postoperative hospitaliza-tion time, complication, and satisfactory rate, especially in postoperative recovery time （12.53 ±2.77d versus 25.95 ±4.98d,t =15.577,P 〈0.001）.But the inhospital cost was higher .Conclusion Laparoscopic appendec-tomy is a good therapy of choice for military patients with acute appendicitis , which can highly reduce non -combat casualty.Meantime several possible methods can be achieved to control the cost .
