内镜黏膜下剥离术在治疗早期胃癌及癌前病变中的应用

Clinical application of endoscopic submucosal dissection in treatment of early gastric cancer and precancerosis

目的研究使用内镜黏膜下剥离术治疗早期胃癌及癌前病变的临床效果。方法将我院收治的40例早期胃癌及癌前病变的患者按其意愿分为对照组和试验组,分别采用常规外科开放手术和内镜黏膜下剥离术实施治疗。观察两组患者的手术时间、手术期间实际出血量、住院期间总费用、术后恢复时间、术后并发症以及术后随访。结果试验组患者的手术时间、手术期间实际出血量、住院期间总费用明显少于对照组;而试验组患者术后恢复时间、术后并发症也明显少于对照组。术后随访半年两组患者均无复发。结论应用内镜黏膜下剥离术治疗早期胃癌及癌前病变临床效果非常显著。

Objective To explore the feasibility and efficacy of endoscopic submucosal dissection （ESD） in treatment of early gastric cancer and precancerosis. Methods 40 patients with early gastric cancer or precancerosis were divided into two groups according to their wishes. The control group was revieved surgical operation and the experimental group was treated with ESD. The surgery time, the actual blood loss, the hospital total cost, postoperative recovery time, postoperative complications and post-operation follow-up were observed between two groups. Results The time of surgery, the actual blood loss, and the hospital total cost of patients in the experimental group were significantly lower than the＇ control group; at the same time, postoperative recovery time and postoperative complications were significantly less than the control group. All patients were followed up, and they were no recun＇ence after surgery in half a year. Conclusion ESD is very safe and effective to treat early gastric cancer and precancerosis.
