疏肝健脾法对功能性消化不良胃肠激素的调节研究

Study on the Regulation of Dispersing Stagnated Liver Qi for Strengthening Spleen Therapy on Gastrointestinal Hormones of Functional Dyspepsia

目的观察疏肝健脾法治疗功能性消化不良(FD)的临床疗效,探讨胃肠激素与FD的关系。方法治疗组选择36例功能性消化不良肝郁脾虚患者,给予疏肝健脾方治疗4周,另设置对照组为20名健康志愿者,观察治疗前后中医症状总疗效、单项症状疗效及胃肠激素(胃动素、生长抑素、胆囊收缩素)情况,结合心理评估报告,并与健康对照组比较。结果治疗组治疗前后中医症状总评分比较有统计学意义(P<0.05);胃动素水平较治疗前升高、生长抑素水平较治疗前降低、促肾上腺皮质激素释放激素水平较治疗前升高水平较治疗前升高(P<0.05),患者抑郁症状减轻。结论疏肝健脾方对功能性消化不良肝郁脾虚型疗效确切,可调节胃肠激素分泌失常,胃肠激素分泌失调在FD发病中起到一定作用。

Objective To observe clinical effect of dispersing stagnated liver qi for strengthening spleen therapy in the treatment of functional dyspepsia（FD）, and to explore the relationship between gastrointestinal hormone and FD. Methods The treatment group of 36 cases of patients with FD of liver depression and spleen deficiency was given dispersing stagnated liver qi for strengthening spleen therapy for 4 weeks. 20 healthy volunteers were selected as the control group. The total efficacy of TCM symptoms, individual symptoms and gastrointestinal hormone（motilin, somatostatin, gallbladder contraction element） before and after treatment were observed, combined with the psychological assessment report, and they were compared with health control group. Results After treatment, the difference on the total score of TCM symptoms had statistical significance（P 〈0.05）. After treatment, the level of motilin increased, growth somatostatin levels decreased, promoting adrenal cortical hormone releasing hormone levels increased（ P 〈0.05）,and depression symptoms in patients reduced. Conclusion The dispersing stagnated liver qi for strengthening spleen therapy in the treatment of FD has exactly curative effect, can regulate gastrointestinal hormone secretion disorders, and gastrointestinal hormone secretion disorders in the pathogenesis of FD plays a certain role.
