优质护理干预在妊娠期糖尿病患者中的应用及对新生儿神经行为发育情况的影响

The influence of high quality nursing intervention on patients with gestational diabe-tes and the situation of neonatal neurobehavioral development

目的：探讨优质护理干预在妊娠期糖尿病患者中的应用及对新生儿神经行为发育情况的影响。方法随机选取我院2012年10月至2014年5月期间收诊的妊娠期糖尿病患者92例，依照其入院顺序分为观察组和对照组各46例。观察组采用优质护理干预，对照组采用常规护理干预，观察两组患者血糖控制情况、妊娠结局（包括自然分娩、产后出血、羊水过多3方面）及患者新生儿的神经行为发育情况（包括行为能力、主动肌张力、被动肌张力、原始反射、一般状态及其总分），并调查患者满意度。结果观察组患者孕妇的血糖控制情况明显优于对照组，差异具有统计学意义（ P〈0.05）。观察组患者自然分娩率及产后出血量均明显优于对照组，差异具有统计学意义（ P〈0.05）；两组患者羊水过多比率差异无统计学意义（ P〉0.05）。观察组患者的新生儿神经行为发育情况明显优于对照组，对护理满意度明显高于对照组，差异均有统计学意义（ P〈0.05）。结论优质护理干预在妊娠期糖尿病患者中的应用临床效果明显，且对新生儿的神经行为发育有着积极重要的影响，值得进一步推广应用。

Objective To explore the application of high quality nursing intervention on patients with gesta-tional diabetes and the situation of neonatal neurobehavioral development. Methods A total of 92 patients with gesta-tional diabetes in our hospital from October 2012 to May 2014 were randomly selected and were divided into observation group and control group according to their orders of admission, 46 cases in each group. The observation group were given the high-quality nursing intervention, while the control group were given routine nursing intervention. The glycemic con-trol, pregnancy outcomes （ including three aspects： natural childbirth, postpartum hemorrhage and polyhydramnios） and neurobehavioral development of newborn patients （ including capacity, active muscle tone, passive muscle tone, primitive reflexes, general state and the overall scores） were observed, and patientsˊsatisfaction of the two groups were investigated. Results The glycemic control of the pregnant women of the observation group was superior than that of the control group, and the difference was statistical significant （ P〈0. 05 ） . Natural birth rate of patients and postpartum hemorrhage of the observation group were significantly better than that of the control group, and the difference was sta-tistically significant （ P〈0. 05 ） . The ratio of polyhydramnios of the two groups showed no statistically significant differ-ence （ P〉 0. 05 ） . The newbornˊs neurobehavioral development of the observation group was better than that of the con-trol group, and the degree of satisfation of the observation group was significantly higher than that of the control group, and the difference was statistically significant （ P〈0. 05 ） . Conclusion The clinical effect of the high quality nursing intervention for patients with gestational diabetes is obvious, and it has a positive and significant role for the newborn neurobehavioral development, which is worth a further promotion and application.
