摘要
目的观察经前期综合征肝气郁证患者抑郁情绪与主要调控脑区的关系。方法收集经前期综合征肝气郁证患者与健康女大学生各15例,分别于月经前3~7天填写抑郁自评量表(SDS)并计算SDS总分及四因子(精神性、躯体性障碍、精神运动性障碍、抑郁的心理障碍)分值;采用功能性磁共振对感兴趣脑区(ROI)激活强度进行分析,对SDS总分及四因子分值和ROI激活强度进行相关性分析。结果病例组额叶脑区激活强度与SDS量表躯体性障碍因子分呈负相关(r=-0.4986,P〈0.05);对照组额叶脑区激活与SDS量表的躯体性障碍分值呈正相关(r=0.3892,P〈0.05)。小脑激活强度与SDS总分及四因子分值未发现相关性。结论 PMS肝气郁证患者SDS躯体性障碍因子与额叶相关性显著,说明PMS肝气郁证患者抑郁情绪与额叶密切相关;抑郁情绪是否受小脑脑区的调控有待于进一步研究。
Objective To study the relationship between depressive mood and primary regulation encephalic region of liver Qi stagnation syndrome patients with premenstrual syndromes( PMS). Methods Fifteen liver Qi stagnation syndrome patients with PMS and 15 healthy female college students were collected. They filled in the Self rating Depression Scale( SDS) on 3- 7 days before menstruation. SDS total scores and 4 factors( mental factor,somatic disorder,psychomotor disturbance and psychological disorder of depression) scores were calculated. Activation intensity of brain regions of interest( ROI) was analyzed by functional magnetic resonance. SDS total scores,scores of 4 factors and ROI activation intensity were made correlation analysis. Results Activation intensity of brain frontal lobe in the patient group negatively related to the somatic disorder factor score of SDS( r =- 0. 4986,P 0. 05). Activation intensity of brain frontal lobe in the control group was positively correlated with the somatic disorder factor score of SDS( r = 0. 3892,P 0. 05). No correlation was found between cerebellum activation intensity and SDS total scores,scores of 4 factors. Conclusion The correlation between PMS liver Qi stagnation syndrome patients' SDS somatic disorder factor and frontal lobe was significant,which meant that depressive mood of PMS liver Qi stagnation syndrome patients was closely related to frontal lobe. Whether depressive mood was regulated to cerebellum region needed further study.
