氟西汀和氟伏沙明治疗首发抑郁症患者的疗效及对其社会功能影响的比较研究

Comparison of effects of Fluoxetine and Fluvoxamine in treatment of patients with first-episode depression and their influence on social function

目的:比较氟西汀和氟伏沙明治疗首发抑郁症患者疗效及对其社会功能的影响。方法:选取80例首发抑郁症患者,随机分为观察组和对照组,每组各40例。观察组患者给予氟西汀治疗;对照组患者给予氟伏沙明治疗。比较治疗前后,采用汉密尔顿抑郁量表(HAMD)评分与社会和工作适应状况问卷(WSAS)评定两组患者的抑郁症状改善情况;采用健康状况问卷(SF-36)测定两组患者的生活质量,包含躯体功能(PF)、生理职能(RP)、躯体疼痛(BP)、总体健康(GH)、生命活力(VT)、社会功能(SF)、情感职能(RE)与精神健康(MH)8个维度;比较两组患者的药物不良反应。结果:治疗后,观察组患者的HAMD评分与WSAS总分显著低于对照组;两组患者的PF、GH、VT、SF、RE与MH评分显著高于对照组,差异均有显著统计学意义(P<0.01);两组患者药物不良反应发生率无明显差异(P>0.05)。结论:氟西汀治疗首发抑郁症患者疗效及对其社会功能改善效果优于氟伏沙明治疗。

Objective： To compare effects of Fluoxetine and Fluvoxamine in treatment of patients with first-episode depression and their influence on social function. Methods ： 80 patients with first-episode depression were selected and randomly divided into observation group and control group. The observation group was given the Fluoxetine treatment, while the control group accepted the Fluvoxamine treatment. Before and after the treatment, the depression improvement was assessed with Hamilton depression scale （HAMD） and WSAS; the quality of life was evaluated with short form 36 health survey questionnaire （ SF-36） , including 8 dimensions such as physical function （ PF ） , physical role （ RP） , body pain （ BP） , general health （ GH） , life vitality （ VT） , social function （ SF ）, emotion role （ RE ） and mental function （ MH） ; and the drug adverse reactions of the two groups were compared. Results ： After the treatment, the HAMD score and WSAS score of observation group were significantly lower than those of control group, the scores of PF , GH, VT, SF , RE and MH were obviously higher than those of control group, and the differences were statistically significant （P 〈0. 01）. There were no significant differences in the drug adverse reaction incidences between the two groups （ P 〉0. 05）. Conclusions： Fluoxetine has better effects in the treatment of the patients with first-episode depression, and can improve their social function more significantly than Fluvoxamine.
