微创经皮肾取石治疗小儿复杂性肾结石疗效分析

Effect Analysis of the Treatment of Children Complex Renal Calculi with Minimally Invasive Percutaneous Nephrolithotomy

目的探讨运用微创经皮肾取石治疗小儿复杂性肾结石的临床疗效。方法将我院68例小儿复杂性肾结石患儿随机分为观察组(34例)和对照组(34例),前者用微创经皮肾取石治疗,后者用传统开放手术治疗,比较患儿手术时间、术中出血量、住院时间、结石清除率、血清肌酐浓度以及并发症发生率。结果观察组手术时间(110.3±18.2)min、住院时间(6.9±1.3)d短于对照组(157±23.4)min、(13.9±3.4)d,术中出血量(251.3±64.5)ml、术后并发症发生率(8.82%)低于对照组(458.5±84.2)ml、(20.6%),而结石清除率(85.3%)高于对照组(61.8%);另外观察组术后4个月、8个月的肌酐浓度(76.5±17.24)umol/L、(82.5±16.3)umol/L低于对照组(134.5±36.7)umol/L、(100.24±21.5)umol/L,且P<0.05。结论运用微创经皮肾取石治疗小儿复杂性肾结石疗效显著,值得在临床上广泛推广及运用。

Objective To discuss clinic effect of the treatment of children complex renal calculi with minimally invasive percutaneous nephrolithotomy. Methods 68 cases of children complex renal calculi were randomly divided into the observation group（34 cases） and the control group（34 cases）, the former group with minimally invasive percutaneous nephrolithotomy treatment, the latter group with traditional open surgery, operative time, blood loss, hospital stay, stone clearance rate, serum creatinine concentration and complication rates were compared. Results Observation operative time（110.3 ± 18.2） min, hospital stay（6.9 ± 1.3） d were shorter than that of the control group（157 ± 23.4） min,（13.9 ± 3.4） d, blood loss（251.3 ± 64.5） ml. The complication rate（8.82 percent） was lower than that of the control group（458.5 ± 84.2） ml（20.6%）, while the stone clearance rate（85.3%） was higher（61.8%）; after 4 months and 8 months,creatinine concentration（76.5 ± 17.24） umol / L,（82.5 ± 16.3） umol / L were lower than that of the control group（134.5 ± 36.7） umol / L,（100.24 ± 21.5） umol / L, and P〈0.05. Conclusion Using minimally invasive percutaneous nephrolithotomy treatment on children with complex renal calculi has significant effect, it should be popularized and used in clinical practice.
