糖尿病夏科足无痛性足底溃疡致截肢一例报告

Painless foot ulcer of Charcot foot lead to amputation:A case report

糖尿病夏科关节病又称糖尿病神经关节病,是一种相对少见的糖尿病慢性并发症,夏科足特指发生在糖尿病患者足部的夏科关节病,好发于DPN的患者,因其临床表现隐匿常被临床医师漏诊或误诊,易导致不良的临床结局。本文报道一例长期被漏诊的糖尿病夏科足患者,足部多个关节骨折移位、畸形,引发足底无痛性溃疡,最终并发严重感染而导致截肢。通过总结该病例的诊治过程旨在复习夏科足的规范化诊疗原则,提高临床对夏科足的诊断率。

Diabetic Charcot joint, also known as Charcot neuro-osteoarthropathy, is a relatively uncommon chronic diabetic complication. Charcot foot is a type of Charcot joint disease happening in the foot of patients with diabetes. It usually occurs in patients with diabetic peripheral neuropathy. Because of its insidious clinical manifestations, Charcot foot is often misdiagnosed or missed diagnosed, and often leads to adverse clinical outcomes. Here,we reported a patient with Charcot foot who was missed diagnosed for a long period. The patient had fracture displacements and deformities that led to plantar painless ulcer. Finally, this patient was amputated due to serious infection. Through summarizing this case and reviewing standardizations of diagnosis and treatment of Charcot foot, we aimed to improve the diagnosis rate of Charcot foot in clinical practice.
