胰十二指肠切除术后早期序贯性肠内营养支持的临床研究 被引量:7

摘要 目的探讨胰十二指肠切除术后早期序贯性肠内营养支持的疗效。方法将45例胰十二指肠切除术后患者按照随机数字表法分为序贯肠内营养支持(SEN)组(n=24)及全肠外营养(TPN)组(n=21)。观察两组患者术后血清总蛋白(TP)、清蛋白(ALB)和前清蛋白(PA)水平以及肛门排气时间、术后住院时间和营养支持费用。结果术后第7天,SEN组患者PA明显升高,术后第10天TP、ALB、PA均明显高于TPN组,患者肛门排气时间和术后平均住院时间明显短于TPN组,术后营养支持费用也明显少于TPN组,差异有统计学意义(P<0.05)。结论胰十二指肠切除术后早期采用SEN支持更有利于改善营养状况,促进术后恢复,降低住院费用。 Objective To compare the clinical effectiveness of sequential enteral nutrition ( SEN) and total parenteral nutri-tion ( TPN) after pancreaticoduodenectomy.Methods Forty-five patients after pancreaticoduodenectomy were randomly divided into SEN group (n=24) and TPN group (n=21).The serum levels of postoperative total protein (TP),albumin (ALB) and prealbumin ( PA) ,the time to anal exhaust,the postoperative hospital stay and nutritional cost were compared between the two groups.Results Compared with the TPN group,there was a significant increase in the serum levels of PA on the 7 th postoperative day.Moreover,on the 10 th postoperative day,the serum levels of TP,ALB and PA were higher in the SEN group than that in the TPN group.Furthermore,the time to anal exhaust,the length of postoperative hospital stay and the cost of nutritional support were less in the SEN group ( P 〈0.05) .Conclusion SEN can greatly improve the nutritional status,promote the postoperative recovery and reduce the expenditure in patients after pancreaticoduodenectomy.
