外痔切剥辅助PPH术治疗混合痔的临床疗效观察

CLINICAL EFFECTS OF EXTERNAL HEMORRHOIDECTOMY COMBINED WITH PPH OPERATION IN THE TREATMENT OF MIXED HEMORRHOID

目的 探讨外痔切剥辅助吻合器痔上黏膜环切钉合术（PPH）对混合痔的临床效果。方法采用随机数字表法将山东枣庄矿业集团枣庄医院接诊的150例混合痔患者随机分成实验组（n=75例）与对照组（n=75例）。实验组采用外痔切剥辅助PPH术进行治疗,对照组患者单纯采用PPH术进行治疗。治疗结束后对两组患者的临床疗效进行评估,同时记录两组患者并发症发生情况及复发率,并进行比较分析。结果 实验组、对照组总有效率分别为97.33%、88.00%,差异有统计学意义（χ~2=6.4035,P=0.0113）;实验组、对照组术后并发症发生率分别是29.33%、52.00,术后复发率分别是0.00%、5.33%,两个率比较差异均有统计学意义（χ~2=10.6498,P=0.0011;χ~2=5.4759,P=0.0192）。结论 对混合痔患者采用外痔切剥辅助PPH术治疗疗效显著,可有效降低患者的术后并发症及复发率,值得临床推广应用。

Objective To explore the clinical effect of external hemorrhoidectomy combined with proce- dure for prolapse and hemorrhoids （PPH） on mixed hemorrhoid： Methods 150 patients with mixed hemor- rhoid in our hospital were ＂randomly divided into the control group （ n =75） and the experimental group （ n =75）. The control group was treated with procedure for prolapse and hemorrhoids （PPH）,while the ex- perimental group was treated with external hemorrhoidectomy combined with PPH. After the treatment, the clinical effeet on the two groups was assessed and the incidence of complications and recurrence of two groups were recorded and comparative analyzed. Results After treatment, the total effective rate was 97. 33% in the experimental group and 88.00% in the control group respectively （x2=6. 4035, P =0. 0113）. The incidence of postoperative complications was 29.33% in the experimental group and 52.00 in the con- trol group respeetively（x2 = 10. 6498, P =0. 0011）. The incidence of recurrence was 52.00% in the experi- mental group and 5.33% in the control group respectively（x2 = 5. 4759, P = 0. 0192）. Conclusion Externalhemorrhoidectomy combined with PPH can produce good curative effect on mixed hemorrhoid by effectively reducing postoperative complications and recurrence.
