循证式督导法在老年肾衰终末期血透患者护理中的应用

Application of evidence-based supervision method in the nursing of elderly patients with end-stage renal failure undergoing hemodialysis

目的观察循证式督导法在老年肾衰终末期血透患者护理中的应用效果。方法研究对象源于我院2013年12月至2014年12月收治的68例肾衰终末期血透老年患者，按随机数字表法分为对照组与观察组各34例。对照组患者接受常规护理；观察组按循证式督导法给予护理。记录两组患者治疗期间失衡综合征、血脂代谢紊乱、透析低血压、深静脉导管感染等并发症情况，比较两组患者护理满意率。结果观察组护理满意率94．1％，高于对照组的76．5％，两组比较差异有统计学意义（P〈0．05）。观察组患者失衡综合征、血脂代谢紊乱、透析低血压、深静脉导管感染发生率（8．8％、2．9％、5．9％、2．9％）均低于对照组，两组比较差异有统计学意义（P〈0．05）。结论循证式督导法在老年肾衰终末期血透患者护理中有良好的应用价值，能有效提高患者护理满意度，减少血透相关并发症的发生。

Objective To observe the application effect of evidence-based supervision method in the nursing of elderly patients with end-stage renal failure undergoing hemodialysis. Methods 68 elderly patients with end-stage renal failure undergoing hemodialysis in our hospital from December 2013 to December 2014 were selected, and divided into control group （34 cases） and observation group （34 cases） according to the random number table. Patients in control group received routine nursing care, while patients in observation group were given nursing according to evidence-based supervision method. The complications in two groups such as disequilibrium syndrome, lipid metabolism disorder, intradialytic hypotension, and deep venous catheter infection during treatment were recorded. The nursing satisfaction rates in two groups were compared. Results The nursing satisfaction rate in observation group was 94.1%, significantly higher than 76.5% in control group, with statistically significant difference between two groups （P〈0.05）. The incidence of disequilibrium syndrome, lipid metabolism disorder, intradialytic hypotensiun, and deep venous catheter infection in observation group （8.8%, 2.9%, 5.9%, 2.9%） were significantly lower than those in control group, with statistically significant differences between two groups （P〈0.05）. Conclusions Evidence-based supervision method is of good application value in the nursing of elderly patients with end-stage renal failure undergoing hemodialysis, which can effectively improve patients＇ satisfaction and reduce the incidence of hemodialysis related complications.
