术前输液对胃腔镜病人术后血糖的影响

Influence of preoperative infusion on postoperative blood glucose in patients with gastric endoscopic surgery

[目的]探讨术前输液对禁食禁饮时间过长的胃腔镜病人血糖的影响,为临床改善术前口渴、饥饿程度和缩短术后血糖恢复时间提供依据。[方法]将122例禁食禁饮超过12h的胃腔镜手术病人随机分为观察组60例和对照组62例。在禁食禁饮达到12h时,观察组用套管针穿刺静脉输入5%葡萄糖生理盐水1 000mL,对照组未输入任何液体。观察并记录两组病人手术前后口渴、饥饿程度及血压、心率、空腹血糖值。[结果]观察组术前口渴、饥饿程度较对照组明显改善(P<0.01)。两组麻醉前收缩压、舒张压、心率比较差异有统计学意义(P<0.05)。两组病人术后空腹血糖均有上升,术后第1天、第3天两组空腹血糖值比较差异有统计学意义(P<0.05)。[结论]术前禁食禁饮时间延长,会增加术后应激性高血糖的发生,采取术前输液能改善病人术前口渴、饥饿程度,缩短术后血糖恢复时间。

Objective:To probe into the influence of preoperative infusion on blood glucose of patients with gastric endoscopy with too long fasting time,so as to provide evidences for clinical improvement of preoperative thirst and hunger and shorten the recovery time of blood glucose.Methods:A total of 122 cases of patients undergoing gastric endoscopic surgery with more than 12 hfasting and were divided into observation group with 60 cases and control group with 62 cases.When fasting reached 12 h,patients in observation group were given intravenous infusion of 5% glucose saline 1 000 mL with trocar puncture,the patients in control group did not receive any liquid.Then thirst,hunger,blood pressure,heart rate and fasting blood glucose of the two groups were observed and recorded.Results:Thirst and hunger degree of patients in observation group was significantly higher than that in control group(P<0.01),There were statistically significant difference in systolic blood pressure,diastolic blood pressure and heart rate between the two groups(P<0.05).The postoperative fasting blood glucose of patients in both groups was increased,there was statistically significant difference in fasting bood glucose on the postoperative 1st day and 3rd day(P<0.05).Conclusion:The duration of fasting before operation could increase the occurrence of stress hyperglycemia after operation,and preoperative infusion could improve the degree of thirst and hunger before operation,and shorten the recovery time of blood sugar.
