经鼻导管湿化高流量吸氧在ICU患者撤机后的应用效果观察 被引量:22

摘要 目的:观察经鼻导管湿化高流量吸氧在ICU气管插管脱机患者中的应用效果。方法:选取我院综合ICU 2014年9月-2015年11月收治的120例气管插管脱机后的患者进行研究。所有患者均经口气管插管接呼吸机辅助呼吸,按照正常程序拔管脱机。脱机后将患者采用随机数字表法分为两组,每组60例,分别给予经鼻导管湿化高流量氧疗(观察组)和普通文丘里面罩吸氧(对照组),观察两组患者在氧合指数、二氧化碳分压、呼吸频率、患者耐受性、舒适度等方面的差别。结果:(1)观察组患者氧合指数高于对照组,二氧化碳分压、呼吸频率低于对照组。(2)观察组患者在舒适度、患者耐受性以及气道湿化情况方面明显优于对照组。结论:ICU气管插管脱机患者应用湿化的高流量鼻导管氧疗能提高其氧合指数、治疗的耐受性,降低再插管率,是一种新的有效的呼吸支持模式。 Objective:To observe the effect of nasal high-flow humidified oxygen therapy in tracheal intubation patients with weaning from ventilation.Methods:One hundred and twenty patients with mechanical ventilation who were admitted in ICU from September 2014 to November 2015 were studied.All cases were orotracheal intubation and weaning with the standard procedure.Patients were randomly divided into two groups.The patients in the experimental group(n=60)received high-flow humidified oxygen therapy.The patients in the control group(n=60)received Venturi mask oxygen therapy.The oxygenation index,partial pressure of carbon dioxide,respiratory rate,tolerance and comfort were compared between the two groups.Results:The oxygenation index of the patients in the experimental group was higher than that of the control group and the partial pressure of carbon dioxide and respiratory rate were lower than that of the control group.The comfort,tolerance and airway humidification of patients in the experimental group were better than that of the control group.Conclusion:In tracheal intubation patients with weaning from ventilation,highflow humidified oxygen therapy is an effective intervention for respiratory with improvement of pulmonary oxygenation and decrease of intubation rate.
