高压氧辅助治疗对卒中后抑郁患者血清5-HT、NE及神经功能的影响 被引量:5

摘要 目的观察高压氧(HBO)辅助治疗对卒中后抑郁(PSD)患者血清5-羟色胺(5-HT)、去甲肾上腺素(NE)及神经功能的影响。方法 70例PSD患者根据数字表法随机分为2组,对照组(n=35例)采用常规措施治疗,观察组(n=35例)待确定活动性出血已稳定或已趋于稳定后,在对照组治疗基础上加用HBO治疗。两组疗程均为30d,比较两组患者治疗前后血清5-HT、NE表达水平及汉密尔顿抑郁量表(HAMD)、中国脑卒中量表(CSS),改良Barthel指数(MBI)评分变化。结果两组患者治疗后血清5-HT、NE表达水平均明显升高(P<0.05),且观察组升高较对照组更为显著(P<0.05);两组患者治疗后HAMD、CSS评分明显降低(P<0.05),而MBI评分明显升高(P<0.05),且观察组降低或升高较对照组更为显著(P<0.05)。结论 HBO辅助治疗可明显升高PSD患者血清5-HT、NE表达水平,改善抑郁状态和神经功能。 Objective To observe the effect of hyperbaric oxygen (HBO) assist therapy on serum 5-hydroxytryptamine (5-HT), nor- epinephrine (NE) and neural function of the patients with post-stroke depression (PSD). Methods 70 patients with PSD were randomly divided into 2 groups: observation group and control group, 35 cases in each; routine therapy was applied to patients in both groups while HBO assist therapy was added to patients in observation group when active bleeding had been controlled or tended to be con- trolled; the courses of the 2 groups both lasted for 30 days; a comparison of the levels of serum 5-HT and NE before and after treatment as well as the score changes of HAMD, CSS and MBI was made between the 2 groups. Results The levels of serum 5-HT and NE of the patients in both groups after treatment increased obviously (P〈0.05) and the increase in observation group was even greater than that in control group (P〈0.05); the scores of HAMD and CSS of the patients in both groups after treatment decreased obviously (P〈0.05) while the scores of MBI increased (P〈0.05), and the decrease and increase in observation group were greater than those in control group (P〈 0.05). Conclusions HBO assist therapy can obviously increase the level of serum 5-HT and NE of the patients with PSD and improve the depression symptoms and the neural function.
