PKP治疗骨质疏松性椎体压缩骨折骨水泥渗漏的相关因素分析

Analysis of related factors of bone cement leakage in the treatment of osteoporotic vertebral compression fracture treated with PKP

目的分析经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折(OVCF)骨水泥渗漏的相关因素。方法回顾性分析我院收治的150例行PKP治疗OVCF患者的临床资料,根据术中有无骨水泥渗漏对其进行分组,其中渗漏组21例,非渗漏组129例,对比分析两组引起骨水泥渗漏的相关因素。结果单因素分析提示渗漏组在术前椎体前中柱平均高度、椎体周壁有无破坏以及骨水泥注入量方面与非渗漏组差异较大(P<0.05);最终多因素分析提示骨水泥注入量及椎体周壁有无破坏与PKP治疗OVCF骨水泥渗漏的发生有密切关系(P<0.05)。结论合理注射骨水泥剂量提高手术操作技术可有效降低PKP手术骨水泥渗漏的发生率。

Objective To analyze related factors of bone cement leakage in the treatment of osteoporotic vertebral compression fracture treated with PKP. Methods The clinical data of 150 cases patients with OVCF who accepted PKP were retrospectively analyzed. According to whether with bone cement leakage, the patients were divided into leakage group （21cases） and non leakage group （129 cases）. Correlation factors of bone cement leakage were compared and analyzed between two groups. Results The single factor analysis showed that the average height of the anterior vertebral column in the front of the vertebral column, whether with damage to the vertebral wall and cement injection volume of two groups had significant difference（P〈0.05）. It was found that there was a close relationship between whether with damage to the vertebral wall, the cement injection volume and the occurrence of PKP bone cement leakage in the treatment of OVCF. Conclusion The occurrence rate of bone cement leakage can be effectively reduced by improving the operation technique of PKP and the proper cement injection volume.
