摘要
目的探讨肺部感染对不稳定性心绞痛(UAP)患者介入术后超敏C-反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)、白细胞介素的影响,以期为临床提供数据依据。方法选择医院2013年1月-2015年3月收治住院且均拟行经皮冠状动脉介入(PCI)治疗的UAP患者216例进行研究,所有患者均给予标准PCI方法治疗,术后7d内46例患者发生感染定为感染组,170例未感染患者定为未感染组,比较两组患者动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO_2)、肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白Ⅰ(cTnI)、hs-CRP、TNF-α、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)测定结果。结果感染组患者PaO2、CK-MB、cTnI低于未感染组,PaCO_2、hsCRP、TNF-α、IL-6、IL-10高于未感染组,差异有统计学意义(P<0.05)。结论 UAP介入术后合并肺部感染可增加hs-CRP、白细胞介素等相关炎症因子及蛋白含量,降低肺功能,加重心肌细胞损害。
OBJECTIVE To explore the influence of pulmonary infection on high‐sensitivity C‐reactive protein (hs‐CRP) ,tumor necrosis factorɑ(TNF‐ɑ) ,and interleukins of patients with unstable angina after interventional sur‐gery so as to provide clinical data .METHODS A total of 216 patients with unstable angina who were hospitalized and received percutaneous coronary intervention (PCI) from Jan 2013 to Mar 2015 were enrolled in the study .All of the patients received the standard PCI ,46 patients who had infections within 7 days after the surgery were as‐signed as the infection group ,and 170 patients who did not have infections were set as the non‐infection group . The partial pressure of arterial oxygen (PaO2 ) ,partial pressure of carbon dioxide (PaCO2 ) ,creatine kinase isoenzyme (CK‐MB) ,cardiac troponin Ⅰ (cTnI) ,hs‐CRP ,TNF‐ɑ,interleukin‐6 (IL‐6) ,and interleukin‐10 (IL‐10) were determined and compared between the two groups of patients .RESULTS The PaO2 ,CK‐MB ,and cTnI were lower in the infection group than in the non‐infection group;the PaCO2 ,hs‐CRP ,TNF‐ɑ,IL‐6 ,and IL‐10 were higher in the infection group than in the non‐infection group ,and there was significant difference (P〈0 .05) . CONCLUSION The postoperative pulmonary infection may lead to the increase of the inflammatory factors such as hs‐CRP and interleukins as well as content of protein ,reduce the lung function ,and exacerbate the myocardial cell damage of the patients with unstable angina after interventional surgery .
