EMP1、EGF和NRG3在鼻息肉中的表达水平及糖皮质激素的影响 被引量:4

摘要 目的探讨上皮膜蛋白1(EMP1)、表皮生长因子(EGF)、神经调节蛋白3(NRG3)在鼻息肉中的表达水平及糖皮质激素对其表达的影响。方法鼻息肉组织标本取自于40例鼻息肉病例。选择30例因鼻中隔偏曲接受鼻内镜手术的病例的下鼻甲黏膜组织作为对照组。40例鼻息肉病例中,伴重度上皮增生14例,伴轻中度上皮增生26例;伴鳞状上皮化生12例,不伴鳞状上皮化生28例。采用实时荧光定量技术(qRT-PCR)、免疫组化方法检测EMP1、EGF和NRG3的表达水平。结果鼻息肉组组织中EMP1、EGF、NRG3 mRNA的表达水平均低于对照组,差异有统计学意义(P<0.05)。鼻息肉组组织中EMP1、EGF、NRG3的表达阳性率均低于对照组,差异有统计学意义(P<0.05)。伴有重度上皮增生鼻息肉组织中EMP1、EGF、NRG3 mRNA的表达水平均低于伴有轻中度上皮增生鼻息肉组织,差异有统计学意义(P<0.05)。伴有鳞状上皮化生鼻息肉组织中EMP1、EGF、NRG3 mRNA的表达水平均低于不伴有鳞状上皮化生鼻息肉组织,差异有统计学意义(P<0.05)。治疗后,鼻息肉组织中EMP1、EGF、NRG3 mRNA的表达水平均较治疗前明显升高,差异有统计学意义(P<0.05)。治疗后,鼻息肉组织中EMP1、EGF、NRG3的表达阳性率均较治疗前升高,差异有统计学意义(P<0.05)。治疗前鼻息肉组织中EMP1与EGF、NRG3的mRNA表达水平均呈正比(r=0.367,P=0.014,r=0.352,P=0.019),EGF、NRG3的mRNA表达水平呈正比(r=0.361,P=0.016);治疗后鼻息肉组织中EMP1与EGF、NRG3的mRNA表达水平均呈正比(r=0.356,P=0.026,r=0.341,P=0.029),EGF、NRG3的mRNA表达水平呈正比(r=0.350,P=0.027)。结论 EMP1、EGF和NRG3在鼻息肉发病及上皮损伤、修复过程中起重要作用。糖皮质激素可通过上调EMP1、EGF和NRG3的表达在鼻息肉的上皮损伤和修复中起积极作用。 Objective To observe the expressions of EMP1,EGF,NRG3 in nasal polyps and the effects of glucocorticoids on their expressions. Methods The nasal polyps tissue specimens were taken from 40 cases of nasal polyps,and the other 30 cases of nasal septum deviation of inferior turbinate mucosa taken by nasal endoscopic surgery were selected as control group. Among 40 cases of nasal polyps,there were 14 cases with severe epithelial hyperplasia,26 cases with mild or moderate epithelial hyperplasia,moreover,there were 12 cases with squamous metaplasia,28 cases without squamous metaplasia. The expression levels of EMP1,EGF and NRG3 were detected by Real-time fluorescence quantitative PCR technology( qRT- PCR),immunohistochemistry,respectively. Results The expression levels of EMP1,EGF,NRG3 mRNA in nasal polyps tissues were significantly lower than those in control group( P〈0. 05). The expression levels of EMP1,EGF,NRG3 mRNA in nasal polyps tissues with severe epithelial hyperplasia were significantly lower than those of nasal polyps tissues with mild or moderate epithelial hyperplasia( P〈0. 05). The expression levels of EMP1,EGF,NRG3 mRNA in nasal polyps tissues with squamous metaplasia were significantly lower than those of nasal polyps tissues without squamous metaplasia( P〈0. 05). After treatment,the expression levels of EMP1,EGF,NRG3 mRNA in nasal polyps tissues were significantly increased,as compared with those before treatment( P〈0. 05). Before treatment and after treatment,the expression levels of EMP1 were positively correlated to those of EGF,NRG3 mRNA( P〈0. 05),and the expression levels of EGF were positively correlated to those of NRG3 mRNA( P〈0. 05). Conclusion EMP1,EGF and NRG3 play an important role in the pathogenesis of nasal polyps and in the process of epithelial damage and repairing. Moreover glucocorticoid can increase the expression levels of EMPl,EGF and NRG3 in nasal polyps and may play a positive role in the process of epithelial damage and repairing.
