65例40岁以下乳腺癌化疗患者更年期症状的评估

Assessment of climacteric syndrome of 65 breast cancer patients under 40 with chemotherapy

目的通过改良Kupperman评分量表,对40岁以下乳腺癌化疗患者进行评估,了解其更年期症状的发生情况。方法收集2013年3月至2015年12月在西安交通大学第一附属医院肿瘤外科诊断的40岁以下女性乳腺癌患者65例,在化疗第6周期结束后进行Kupperman评分,描述性分析其更年期综合征的发生状况。结果在65例女性乳腺癌患者中,有53例患者出现了更年期综合征,发生率为81.54%（53/65）;症状程度为中、重度者占33.85%（22/65）,轻度者占47.69%（31/65）。40岁以下女性乳腺癌患者化疗后常见的更年期症状排名前3位为疲乏、失眠和情绪波动,发生率依次为70.77%、56.92%、50.77%。经单因素分析显示,手术方式可能与年轻女性乳腺癌化疗患者发生乳腺癌更年期综合征有关（χ~2=4.472,P〈0.05）。结论 40岁以下女性乳腺癌患者化疗后更年期综合征的发病率较高,需采取有效措施缓解症状,提高其生活质量。

Objective To evaluate the young breast cancer patients under 40 with chemotherapy through Kupperman scale and to understand their climacteric syndrome. Methods Totally 65 cases of breast cancer（ under 40 years of age） diagnosed in the First Affiliated Hospital of Xi＇an Jiaotong University during March 2013 to December 2015 were selected,and Kupperman score was conducted at the end of sixth cycle of chemotherapy. The incidence of climacteric syndrome was descriptively analyzed. Results In 65 cases,81. 54%（ 53/65）suffered climacteric syndrome,including 33. 85%（ 22/65） cases with moderate and severe symptoms and 47. 69%（ 31/65） cases with mild symptoms. The top three climacteric symptoms of cases of breast cancer after chemotherapy were fatigue,insomnia and mood swings,and the incidence was 70. 77%,56. 92% and 50. 77%,respectively. Univariate analysis showed that surgery method might be associated with the occurrence of climacteric syndrome in young breast cancer patients with chemotherapy（ χ~2= 4. 472,P〈0. 05）. Conclusion The incidence of climacteric syndrome is relatively high in young cases of breast cancer after chemotherapy. Effective measures are needed to relieve symptoms and improve quality of life.
