医联体同质化应用于社区糖尿病管理的研究探索

Research and Exploration on the Application of Regional Medical Commonwealth Homogenization in the Management of Diabetes in Community

目的:探讨医联体同质化管理模式在社区糖尿病管理中的应用,以期创建一个新型有效的社区慢病管理模式。方法:采用常规研究方法,将360例糖尿病患者分为干预组180例和对照组180例,分别进行管理,比较效果。结果:干预组糖尿病患者的各项控制指标明显优于对照组,社区医护人员的专科业务能力及健康教育水平得到提升(P<0.05)。结论:通过探索医联体同质化管理模式在社区糖尿病管理中的应用,利用三级综合医院的医疗优势,对社区糖尿病患者进行规范化管理,增强了社区医护人员的慢病管理能力,为其他慢病管理积累了经验。

Objective：To explore the application of regional medical commonwealth homogenization in the management of diabetes in community,only in this way can we create a new effective model of community chronic disease management.Method：Using conventional research methods,360 patients with diabetes were divided into the intervention group（n=180） and the control group（n=180）,the effects of two groups were compared.Result：The control index of the intervention group was significantly better than those of the control group,the community health service ability and the level of health education were improved（P〈0.05）.Conclusion：By exploring the application of regional medical commonwealth homogeneity management model in community management of diabetes patients using tertiary general hospital in the medicare advantage,for the standardized management of community patients with type 2 diabetes,enhance the community health workers ability chronic disease management,for other chronic disease management has accumulated experience.
