风湿性心脏病二尖瓣狭窄患者左心室纵轴功能检测及其临床意义 被引量:1

摘要 目的:分析左心室纵轴功能检测在风湿性心脏病二尖瓣狭窄(MS)中的应用价值。方法:选取2015年1月-2016年5月就诊于本院的30例健康体检者和90例MS患者作为观察对象,采用分层随机抽样法将健康体检者为对照组,以MS患者为MS组,对比两组左心室纵轴功能。同时根据二尖瓣狭窄程度,将MS组患者分为轻度狭窄组、中度狭窄组及重度狭窄组,对比三组左心室纵轴功能,并分析二尖瓣狭窄程度与左心室纵轴功能的相关性。结果:MS组二尖瓣环后间壁房室平面位移值(D_(AV-PS))、二尖瓣环侧壁房室平面位移值(D_(AV-L))、二尖瓣环前壁房室平面位移值(D_(AV-A))、二尖瓣环下壁房室平面位移值(D_(AV-I))、二尖瓣环后间壁收缩期峰值速度(V_(MA-PS))、二尖瓣环侧壁收缩期峰值速度(V_(MA-L))、二尖瓣环前壁收缩期峰值速度(V_(MA-A))及二尖瓣环下壁收缩期峰值速度值(V_(MA-I))均低于对照组,比较差异均有统计学意义(P<0.05)。MS不同狭窄程度三组间D_(AV-PS)、D_(AV-L)、D_(AV-A)、D_(AV-I)、V_(MA-PS)、V_(MA-L)、V_(MA-A)及V_(MA-I)比较差异均有统计学意义(P<0.05),以轻度组最高,重度组最低。MS各组患者中,D_(AV-PS)、D_(AV-L)、D_(AV-A)、D_(AV-I)、V_(MA-PS)、V_(MA-L)、V_(MA-A)及V_(MA-I)与瓣口面积呈正向直线相关性,比较差异均有统计学意义(P<0.05)。结论:MS可导致患者左心室纵轴功能显著下降,且下降程度与狭窄程度存在显著的正向直线相关性。 Objective:To discuss the application value of Left ventricular longitudinal axis function in Rheumatic heart disease Mitral stenosis.Method:From January 2015 to May 2016,30 cases of physical examination and 90 cases of MS in our hospital were selected as research object by randomly,physical examination for the control group,patients with MS for the MS group.Compared the Left ventricular longitudinal axis function of two groups.At the same time,according to the degree of mitral stenosis,MS patients were divided into mild group,moderate group and severe group.Compared the Left ventricular longitudinal axis function of three groups.Analyzed the degree of mitral stenosis and left ventricular function of the vertical axis.Result:The DAV-PS,DAV-L,DAV-A,DAV-I,VMA-PS,VMA-L,VMA-A and VMA-I of MS group were lower than control group,the differences were statistically significant(P〈0.05).The DAV-PS,DAV-L,DAV-A,DAV-I,VMA-PS,VMA-L,VMA-A and VMA-I in MS degree of stenosis were compared differences between three groups had statistical significance(P〈0.05),the mild group was the highest,the severe group was the lowest.And the MS group positively to the linear correlation with DAV-PS,DAV-L,DAV-A,DAV-I,VMA-PS,VMA-L,VMA-A,VMA-I and mitral valve area,the differences were statistically significant(P〈0.05).Conclusion:MS can cause with a significant reduction in the left ventricular function of the vertical axis,and the drop degree there are significantly positive linear correlation with the degree of stenosis.
