软瘫期给予胫前肌及腓骨长短肌电针针刺疗法预防脑卒中后出现足下垂、足内翻的临床研究 被引量:14

摘要 目的 探讨软瘫期电针针刺胫前肌及腓骨长短肌配合现代康复疗法预防脑卒中后出现足下垂及足内翻的临床疗效。方法 100例软瘫期脑卒中患者随机分成实验组对照组两组,每组50例,对照组进行常规康复治疗及患侧下肢针刺足三里、阳陵泉、申脉、解溪、太溪、丰隆、照海、悬钟、承山、太冲等穴位,并选其中足三里-丰隆、悬钟-太冲两组穴位加用电针综合治疗仪进行刺激,周一至周六每日一次。实验组进行常规康复治疗和分别用电针针刺胫前肌及腓骨长短肌,其中周一周四电针针刺胫前肌,周二,三,五,六电针针刺腓骨长短肌。实验组和对照组电针综合治疗仪均选用20~30Hz的连续波进行刺激,每次30min。经过3个月的治疗后的观察两组足下垂发生率和足内翻发生率。结果 实验组足下垂发生率明显低于对照组(P〈0.05)实验组足内翻发生率明显低于对照组(P〈0.05)。结论 软瘫期给予胫前肌及腓骨长短肌电针针刺配合现代康复疗法可有效预防脑卒中后出现足下垂和足内翻。 objective To investigate the clinical effect of the electro-acupuncture therapy on the anterior tibial muscle and the peroneus longus and brevis muscles of flaccid paralysis patient combined with modern rehabilitation therapy to prevent the foot-drop and strephenopodia after cerebral apoplexy. Methods 100 patients of flaccid paralysis were randomly divided into experimental group and control group,50 cases in each group. The control group was given routine rehabilitation treatment and lower limb acupuncture at Zusanli, Yanglingquan,Shenmai,JieXi,Tai xi,Fenglong,Zhaohai,Xuanzhong, Chengshan,and Taichong acupoint, once a day from Monday to Saturday.Zusanli- Fenglong acupoint and Taichong-Xuanzhong acupoint were stimulated by electroacupuncture combined therapeutic instrument. The experimental group was treated with routine rehabilitation and electro acupuncture respectively at the anterior tibial muscle and peroneus longus and hrevis muscles, which were treated with acupuncture on Monday and Tuesday. The length of the length of the leg was three, and five or six respectively.On Monday and Thursday, the anterior tibial muscle was electro acupuncture, and the peroneus longus and brevis muscles was electro acupuncture at Tuesday, Wednesday, Friday, and Saturday. Results The foot-drop incidence rate of experimental group was significantly lower than that of the control group (P 〈 0.05).The incidence rate of foot inversion in the experimental group was significantly lower than that in the control group. Conclusion The electro-acupuncture therapy on the anterior tibial muscle and the peroneus longus and brevis muscles of flaccid paralysis patient combined with modern rehabilitation therapy can effectively prevent the foot-drop and strephenopodia after stroke.
