幽门螺杆菌相关性胃病中医体质与中医证型的关系研究

Relationship between TCM Constitution and TCM syndrome types of Helicobacter pylori associated gastric diseases

目的:探讨Hp相关性胃病中医体质与中医证型的关系。方法:选取350例Hp相关性胃病患者为观察对象,按《中医体质分类与判定》分别判定为9种体质类型,按《中医病证诊断疗效标准》中胃脘痛的有关内容辨证分型为脾胃虚寒证、胃阴亏虚证、肝胃不和证、脾胃湿热证、瘀阻胃络证,分析患者中医体质与证型的关系。结果:1Hp相关性胃病患者中医体质类型出现的频次为湿热质>气郁质>阳虚质>气虚质>平和质>血瘀质>痰湿质>阴虚质>特禀质;2中医证型频次为脾胃湿热证>脾胃虚寒证>肝胃不和证>胃阴亏虚证>瘀阻胃络证;3胃镜征象、组织病理学表现为充血红斑、水肿多见于阳虚质,糜烂多见于痰湿质,阳虚质、气郁质的炎症程度较重,阳虚质、气虚质的萎缩程度较重,气虚质伴肠化生多见;4脾胃虚寒证属阳虚质的较多见,脾胃湿热证属湿热质的较多见,瘀阻胃络证属血瘀质的较多见,胃阴亏虚证属气虚质的较多见,肝胃不和证属气郁质的较多见。结论:湿热质、气郁质、阳虚质和气虚质是Hp相关性胃病的主要体质类型,因体质具有可调性,故对易感体质进行早期干预,可望成为防治Hp相关性胃病的有效方法和途径。

Results 1, Hp associated gastric disease patients with TCM constitution type frequency for damp heat matter 〉 gas stagnation 〉 〉 of Yang deficiency constitution Qi matter 〉 flat and matter 〉 blood stasis 〉 phlegm matter 〉 Yin deficiency 〉 Special intrinsic quality. 2, TCM syndrome type frequency of spleen stomach damp heat syndrome of spleen deficiency syndrome 〉 〉 〉 stomach disharmony between the liver and stomach yin deficiency on gastric blood stasis syndrome. 3, gastroscopie and histopathologie study performance for congestive erythema, edema is more common in Yang deficiency, erosion is more common in phlegm dampness constitution ; degree of inflammation of Yang deficiency, qi stagnation heavier; Yang deficiency, Qi deficiency degree of atrophy of the heavier; Qi deficiency accompanied with intestinal metaplasia. 4, spleen and stomach deficiency cold syndrome is a Yang more see, taste hot and damp card is a hot and humid more see, gastric collat-erals syndrome and blood stasis resistance belong to blood stasis more see, the stomach qi deficiency and Yin deficiency syndrome is more see, liver stomach with card belongs to qi depression more see. Conclusion Hot and humid, qi depression, Yang deficiency and blood stasis type of mass is the main constitution Hp correlation tummy bug. Because the constitution is adjustable to early intervention of susceptible, is expected to become effective ways and means for the prevention and control of Hp correlation tummy bug.
