摘要 目的探究大剂量氨溴索在结核病中的应用。方法选取68例肺结核病患者为研究对象,按照随机数表法将所有患者分为观察组和对照组,每组各34例患者。两组患者均进行解痉、抗感染等常规治疗,在此基础上对照组每天1次静脉滴注氨溴索,每次60-120mg,观察组每天1次静脉滴注氨溴索,每次600mg。比较治疗后2个月、4个月、6个月两组患者痰菌转阴率、空洞吸收情况。结果观察组显效11例(32.4%)、有效20例(58.9%)、无效3例(8.8%)、总有效率91.2%,对照组显效8例(23.5%)、有效17例(50.0%)、无效9例(26.5%)、总有效率73.2%。治疗2个月、4个月、6个月后观察组痰菌转阴率和空洞吸收情况均高于对照组。两组数据比较具有统计学差异(P<0.05)。结论大剂量氨溴索能够提高肺结核患者的治疗效果,降低并发症的发生率,临床推荐用量600mg/次,1次/d。 Objective To explore the application of large dose of ambroxol hydrochloride in tuberculosis. Methods 68 patients with pulmonary tuberculosis were selected, and were randomly divided into the observation group and the control group, 34 patients in each group. All patients were treated with antispasmodic, anti-infection and other conventional treatment. On the basis, the control group was given once daily intravenous ambroxol, 60- 120rag each time, and the observation group was given once daily intravenous ambroxol, 600rag each time. Their sputum negative conversion rate, cavity absorption and symptoms were compared between the two groups 2, 4 and 6 months after the treatment. Results In the observation group, there were 11 markedly effective cases (32. 4% ), 20 effective cases (58.9%), and 3 ineffective cases (8. 8% ), and the total effective rate was 91.2%. In the control group, there were 8 markedly effective cases (23.5%), 17 effective cases (50%) and 9 ineffective cases (26. 5% ), and the total effective rate was 73.2%. 2, 4 and 6 months after the treatment, there were significant differences in the sputum negative conversion rate and cavity absorption between the two groups (P 〈 0.05 ). Con- clusion Large dose of ambroxol in patients with pulmonary tuberculosis can improve the therapeutic effect and re- duce the incidence of complications. The clinical recommended dosage is 600mg/time, once per day.
