新生儿改良式断脐及护理方法的效果研究

Study on the effect of improved umbilical cord and nursing method in neonates

目的：探讨新生儿改良式断脐及护理方法即晚断脐改良纱布包扎后暴露护理法用于新生儿脐部护理的效果。方法：选择2015年7—2015年12月足月分娩的100例新生儿为观察组，出生后脐部处理采取晚断脐（脐带搏动停止后断脐），脐夹断脐后，脐部改良式包扎24~48 h，取下脐夹暴露直至脐带干燥自然脱落。选择2015年1—2015年6月足月分娩的100例新生儿为对照组，出生后脐部处理采取常规断脐法，即早断脐（出生后10 s 内），脐夹断脐后，脐部采取护脐卷包裹24~48 h，取下脐夹仍用护脐卷包裹直至脐带自然干燥脱落。比较观察组与对照组脐带自脱时间，脐部渗血、感染发生的情况。结果：观察组新生儿脐带自然脱落3~7 d 者70例，8~14 d 者21例，超过15 d 者9例。对照组3~7 d 者40例，8~14 d 者43例，超过15 d 者17例。观察组新生儿脐带自然脱落所需时间短于对照组（P 〈0.05）；观察组发生脐部感染、脐部渗血3例，对照组13例，观察组少于对照组，差异有统计学意义（P 〈0.05）。结论：新生儿改良式断脐及护理法可有效缩短新生儿脐带脱落时间，降低新生儿脐部感染、脐部渗血的发生率。

Objective：To explore the nursing effect of neonatal umbilical nursing method and late omphalotomy modified gauze after exposure to nursing method for neonatal umbilical. Methods：From July to December in 2015,100 cases neonatal full- term delivery in observation group,after the birth of the umbilicus treatment take omphalotomy late（after the cessation of umbilical cord pulsation omphalotomy）,umbilical clamp after modified 24 ~ 48 hours from Umbilical Dressing until the umbili-cal cord clip exposed natural drying method off. From January to June in 2015,100 cases neonatal full - term birth in control group after treatment by conventional umbilical omphalotomy method,namely early omphalotomy（within 10s after birth）,um-bilical pinch off after umbilical umbilical cord to roll wrapped 24 ~ 48 hours umbilical clip still use umbilical cord wrapped cord until natural drying fall off. the removal time of umbilical cord,bleeding and the occurrence of infection was compared be-tween two group. Results：In the observation group of neonatal umbilical cord falls off the time required for 3 ~ 7 days of 70 ca-ses,8 ~ 14 days in 21 cases,more than 15 days of 9 cases,the control group of neonatal umbilical cord falls off the time re-quired for ~ 7 days of 40 cases,8 ~ 14 days 43 cases,more than 15 days of 17 cases,the neonatal umbilical cord natural shed-ding required time in observation group shorter than the control group（P 〈 0. 05）. In observation group,the incidence of um-bilical infection,umbilical bleeding complications was 3 cases and 13 cases in control group,observation group is less than con-trol group,the difference has statistical significance（P 〈 0. 05）. Conclusion：neonatal umbilical nursing and improved method can effectively shorten the time of neonatal umbilical,reduce neonatal umbilical infection,incidence of umbilical bleeding.
