摘要
目的:探讨纤维环穿刺诱导椎间盘退变动物模型围手术期应对策略的可行性。方法:健康新西兰大白兔80只,随机分为4组,青霉素禁食组(A组),青霉素不禁食组(B组),非青霉素禁食组(C组),非青霉素不禁食组(D组)。用持针器夹持18G皮肤穿刺针从左前外侧刺入L3/4、L4/5、L5/6椎间盘的纤维环,深度控制在5mm。术后按照分组方法饲养,观察各组术后1周的预后情况。结果:围手术期不予特殊饮食管理的B、D组均出现较多肠梗阻,同时出现总计3只死亡,而A、C组出现肠梗阻只数显著少于另外2组(P<0.05),且无死亡。使用抗生素组与不使用抗生素组对术后切口感染坏死情况的发生无统计学差异。结论:纤维环穿刺诱导椎间盘退变动物模型围手术期的应对策略中,术前、术后的饮食管理可达到降低肠梗阻率,减少死亡率,从而降低研究成本的目的,而术后抗生素的使用与否并不影响预后。
Objective: To explore the feasibility of perioperative strategies of intervertebral disc degeneration animal model induced by annulus puncture. Methods: Eighty healthy New Zealand white rabbits were randomly divided into four groups. Group A were given penicillin and fasting. Group B were treated with penicillin and without fasting. Group C were treated without penicillin but fasting. Group D were given without penicillin and without fasting. The L3/4, L4/5 and L5/6 lumbar intervertebral discs of 80 New Zealand white rabbits were inserted by 18-gauge hypodermic needle into a depth of 5mm in the left anterolateral annulus fibrosis. After surgery, each group rabbits were rearing and medication according to the grouping method, and the prognosis was observed after one week. Results: More cases of intestinal obstruction were occurred in perioperative group B and D without fasting with a total three deaths. The number of intestinal obstruction cases in group A and C was significantly smaller than the other two groups without cases of death(P〈0.05). There was no significant difference between groups treated with antibiotic and groups treated without antibiotics for postoperative infection of necrosis. Conclusion: In perioperative strategies of intervertebral disc degeneration animal model induced by annulus puncture, preoperative and postoperative dietary management can reduce the incidence of intestinal obstruction, and reduce mortality, so as to achieve the purpose of reducing the cost of research, but the use of antibiotics after surgery does not affect the prognosis.
