醒脑静注射液对脓毒症休克兔肠系膜微循环的影响 被引量:4

摘要 目的观察醒脑静注射液对脓毒症休克新西兰大白兔肠系膜微循环的影响。方法将30只新西兰大白兔随机抽取10只入正常对照组,剩余20只全部给予脂多糖1.0 mg/kg的剂量进行内毒素休克造模,造模成功后随机分为实验对照组、醒脑静组。造模后醒脑静组沿耳缘静脉持续滴入醒脑静注射液5 m L/(kg·h),正常对照组及实验对照组滴入0.9%氯化钠溶液5 m L/(kg·h)。分别在实验的3 h、6 h通过BI-2000医学图像系统观察并记录选定部位微循环毛细血管动静脉血色、管径、流速、流态、毛细血管网交点计数变与化,并监测0 h、3 h、6 h心率、平均动脉压、血清白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)变化。结果正常对照组比较,实验对照组微静脉、微动脉管径明显收缩,血流速度减慢,毛细血管交叉网点开放数明显减少,心率、IL-8、TNF-α值明显升高,血压明显降低,血液流态以线粒流、粒缓流为主,少数为粒摆流甚至出现血流停滞,差异有统计学意义(P<0.01);与实验对照组比较,治疗6 h后,醒脑静组微静脉、微动脉管径明显扩张,血流速度增快,毛细血管交叉网点开放数明显增多,心率、IL-8、TNF-α明显下降,血压显著升高,血液流态显著改善,以线流、线粒流为主,差异有统计学意义(P<0.01)。结论醒脑静注射液能显著改善脓毒症休克新西兰大白兔肠系膜微循环灌注,降低IL-8、TNF-α水平。 Objective: To observe the effects of Xingnaojing Injection on the mesenteric microcirculation of rab- bits with septic shock. Methods: 10 New Zealand white rabbits were randomly selected as group A (normal control group) of 30 rabbits,and the remaining 20 rabbits as model group. The rabbit models of septic shock were established by injecting lipopolysaccharide (LPS) through ear vein (1.0 mg/kg). When models were established, they were randomly divided into 2 groups: group B(experimental control group) and group C(Xingnaojing Injec- tion group). After models were established,the rabbits in group C were given Xingnaojing Injection 5 mL/(kg, h) through ear vein,normal control group and experimental control group were given equivalent saline 5 mL/(kg, h). Then the opening number of mesenteric artery and vein diameter,velocity,flow pattern,junction of capillary net- work of rabbits with septic shock at 3 h, 6 h by BI-2000 image analysis system were observed and recorded. And the change of heart rate,mean arterial pressure,tumor necrosis factor (TNF-α),IL-8 level at 0 h,3 h,6 h were observed and recorded. Results: Compared with the normal control group ,the diameter of the micro vein and micro artery shrank significantly,blood flow velocity decelerated,junction of capillary network opening number reduced obviously, and the heart rate, IL-8, TNF-α values increased significantly, blood pressure decreased, and the blood flow pattern mainly presented the line particle flow and the slow particle flow,some presented pendulum flow even for blood stagnation in the experimental control group. The differences were statistically significant (P〈0.01 ). Compared with the experimental control group,the diameter of the micro vein and micro artery dilated sig- nificantly,blood flow velocity accelerated,junction of capillary networkopening number increased obviously,and the heart rate, IL-8 ,TNF-α values decreased significantly,blood pressure increased,blood flow pattern was sig- nificantly improved and mainly presented the line particle flow and particle flow in the Xingnaojing Injection group after 6 h. The differences were statistically significant (P〈0.01). Conclusion: Xingnaojing Injection can sig- nificantly improve the mesentery microcirculation in rabbits with septic shock, and decrease IL-8, TNF-α levels.
