中西医结合治疗糖尿病合并慢性肾功能衰竭临床研究

Clinical Study on Combination of Chinese and Western Medicine in the Treatment of Patients with Diabetes Mellitus Complicated with Chronic Renal Failure

目的:观察黄芎汤联合双嘧达莫治疗糖尿病合并慢性肾功能衰竭患者的临床疗效。方法:80例糖尿病合并慢性肾功能衰竭患者按照随机数字表法将其分为对照组和观察组各40例。对照组给予双嘧达莫,观察组在对照组基础上给予黄芎汤。观察两组患者临床疗效、血流动力学指标、内皮素-1(endothelin-1,ET-1)、尿白蛋白(urinary albumin,UAER)、尿素氮(urea nitrogen,BUN)、空腹血糖(fasting blood-glucose,FBG)、血肌酐(serum creatinine,Scr)指标变化以及不良反应发生率。结果:治疗后观察组有效率为92.50%,显著高于对照组的72.50%(P<0.05);治疗后两组患者血流动力学指标均较治疗前有所下降(P<0.05),且观察组显著低于对照组(P<0.05);治疗后两组患者ET-1、UAER、BUN、FBG、Scr指标均有所降低(P<0.05),且观察组显著低于对照组(P<0.05);观察组不良反应发生率为10.00%,显著低于对照组的22.50%(P<0.05)。结论:黄芎汤联合双嘧达莫治疗糖尿病合并慢性肾功能衰竭可有效改善患者肾功能和血流动力学相关指标,降低不良反应发生率,临床疗效显著。

Objective： To observe the clinical effect of Huang Qiong Decoction combined with dipyridamole in the treatment of patients with diabetes mellitus complicated with chronic renal failure. Methods： Eighty cases of patients confirmed diabetes mellitus complicated with chronic renal failure in our hospital were selected as the research subjects,who were randomly divided into control group and experimental group,with forty cases in each group. The control group were treated with dipyridamole,while the experimental group were treated with Huang Qiong Decoction based on the control group. The clinical treatment effect,changes of hemodynamic parameters,and endothelin-1（ ET-1）,urinary albumin excretion rates（ UAER）,blood urea nitrogen（ BUN）,fasting blood glucose（ FBG）,serum creatinine concentration（ Scr） levels and incidence rates of adverse reactions in the two groups were observed. Results： The effective rate in the experimental group was 92. 50%,significantly higher than that of 72. 50% in the control group（ P〈0. 05）. The hemodynamic parameters in the two groups after treatment were reduced compared with those beforetreatment（ P〈0. 05）,which in the experimental group were significantly lower than those in the control group（ P〈0. 05）. The levels of ET-1,UAER,BUN,FBG,Scr in the two groups after treatment were reduced（ P〈0. 05）,while in the experimental group were significantly lower than those in the control group（ P〈0. 05）. The incidence rate of complications in the experimental group was 10. 00%,which was significantly lower than that of 22. 50% in the control group（ P〈0. 05）. Conclusion： Huang Qiong Decoction combined with dipyridamole in the treatment of patients with diabetes mellitus complicated with chronic renal failure can effectively improve the renal function and hemodynamics related indicators,reduce the incidence of adverse reactions and improve the clinical treatment effect.
