宫腔镜联合补肾调经方治疗宫腔粘连临床研究

Clinical Study of Hysteroscopy Combined with Bushentiaojingfang in Treatment of Intrauterine Adhesions

目的:观察宫腔镜配合补肾调经方治疗宫腔粘连的临床疗效。方法:将108例人流术后月经量减少的患者随机分为对照组和治疗组,每组54例。对照组对于轻度粘连的患者采用硬性宫腔镜的尖端进行推压分离,对于中、重度粘连者,采用电切分离,术后预防性应用抗生素3 d。中、重度粘连患者术后服用补佳乐及安宫黄体酮治疗;治疗组在对照组治疗的基础上联合口服补肾调经方治疗,两组患者均治疗3个疗程。治疗后比较两组患者月经情况、宫腔内壁恢复情况和子宫内膜厚度。结果:治疗组在促进月经恢复、防止宫腔再次粘连方面明显优于对照组,两组比较,差异均有显著性意义(P<0.05);治疗组子宫内膜厚度明显高于对照组,差异有统计学意义(P<0.05)。结论:宫腔镜配合补肾调经方治疗宫腔粘连疗效显著,且能促进宫腔粘连患者术后月经恢复、子宫内膜生长,并有防止复发的作用。

Objective： To observe the clinical curative effect of hysteroscopy combined with Bushentiaojingfang in treatment of uterine cavity adhesion. Methods： 108 cases of postoperative menstrual flow reduction in patients were randomly divided into control group and treatment group,each group of 54 cases. In the control group,the patients with mild adhesion were separated by rigid hysteroscopy. The patients with moderate or severe adhesion were separated by electric resection,and the prophylactic use of antibiotics was used for 3 days after operation,and progynova and medroxyprogesterone acetate were given for moderate or severe adhesion patients. The treatment group was given orally administration Bushentiaojingfang based on the control group,a total of three courses of treatment. After treatment,menstruation,the inner wall of the uterine cavity recovery and endometrial thickness were compared between the two groups. Results： In the treatment group,promoting menstruation recovery,preventing uterine cavity again adhesion were significantly better than the control group,and the differences were significant（ P〈0. 05）; endometrial thickness of the treatment group was significantly higher than the control group,and the difference was statistically significant（ P〈0. 05）. Conclusion： Hysteroscopy combined with Bushentiaojingfang in treatment of intrauterine adhesions has distinct curative effect,which can promote uterine cavity adhesion postoperative recovery of menstruation,uterine endometrial growth and prevent recurrence.
